MEDICAL, HOSPITAL & DRUG INSURANCE. 14.1 The Board shall pay one hundred (100%) per cent of the current premium costs to provide each Member covered by this Agreement with the following: 14.2 Deleted effective January 1, 2006. 14.3 Supplementary Manulife for hospital care for semi-private coverage or equivalent. 14.4 Effective on the first day of the month following settlement, drug prescription coverage pursuant to the Manulife Formulary 3 Plan, subject to a maximum dispensing fee of $7.50 per prescription, and subject to the condition that Members use generic drugs unless the specific generic drug is not effective. 14.5 Extended Health Benefits under the Service’s Group Insurance Plan or equivalent with prosthetic appliances and durable medical equipment to a maximum of twenty thousand ($20,000.00) dollars per person per year, private duty nursing to a maximum of thirty-five thousand dollars ($35,000.00) per person per year, chronic care coverage in the amount of three dollars ($3.00) maximum per day to a maximum of 120 days per person per year and for all other hospital stays private hospital room coverage to a maximum of 120 days per person per year. This benefit shall also provide for para-medical coverage, which includes the following: 14.5.1 Chiropractor, Osteopath, Podiatrist, Chiropodist, Acupuncture and Naturopath to a maximum of four hundred and fifty dollars ($450.00) per benefit year, increasing to five hundred dollars ($500.00) per benefit year effective June 27, 2013, increasing to five hundred fifty dollars ($550.00) per benefit year effective January 1, 2015. The Chiropractic benefit is increased to six hundred dollars ($600.00) effective July 27, 2017, six hundred and fifty dollars ($650.00) effective January 1, 2018, and seven hundred dollars ($700.00) effective January 1, 2019. 14.5.2 Psychologist, psychotherapist and psychoanalyst to a maximum of one thousand dollars ($1,000.00) per benefit year, increasing to one thousand two hundred and fifty dollars ($1,250.00) per benefit year effective June 27, 2013, increasing to one thousand five hundred dollars ($1,500.00) per benefit year effective January 1, 2016, with no hourly fee cap or fee limitation for the first visit. The psychologist, psychotherapist and psychoanalyst benefit is increased to three thousand dollars ($3,000.00) effective January 1, 2018. Retirees are entitled to receive a psychologist benefit of one thousand five hundred dollars ($1,500.00) for two (2) years following retirement effective April 11, 2017. ...

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Health Care Insurance While a faculty member is on an approved leave of this type, the faculty member will be advised regarding the right to continue health care benefits in accordance with COBRA during the period of unpaid absence.

  • Medical Care The Parents must comply with the School Welfare Officer's recommendations which may include a reasonable decision to release the Pupil home or to his / her education guardian when s/he is unwell.

  • Hospital This plan covers behavioral health services if you are inpatient at a general or specialty hospital. See Inpatient Services in Section 3 for additional information. This plan covers services at behavioral health residential treatment facilities, which provide: • clinical treatment; • medication evaluation management; and • 24-hour on site availability of health professional staff, as required by licensing regulations. This plan covers intermediate care services, which are facility-based programs that are: • more intensive than traditional outpatient services; • less intensive than 24-hour inpatient hospital or residential treatment facility services; and • used as a step down from a higher level of care; or • used a step-up from standard care level of care. Intermediate care services include the following: • Partial Hospital Program (PHP) – PHPs are structured and medically supervised day, evening, or nighttime treatment programs providing individualized treatment plans. A PHP typically runs for five hours a day, five days per week. • Intensive Outpatient Program (IOP) – An IOP provides substantial clinical support for patients who are either in transition from a higher level of care or at risk for admission to a higher level of care. An IOP typically runs for three hours per day, three days per week.

  • Extended Health Care Plan ‌ The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable extended health care plan.

  • Medical Insurance The Company shall provide to Executive, Executive's spouse and children, at its sole cost, such health, dental and optical insurance as the Company may from time to time make available to its other executive employees.

  • Medical Examination Where the Employer requires an employee to submit to a medical examination or medical interview, it shall be at the Employer's expense and on the Employer's time.

  • Medical There shall be an open enrollment period for medical coverage in each year of this Agreement. An employee may elect no medical coverage during any open enrollment period. An employee who has elected no medical coverage may elect medical coverage during an open enrollment period. No pre-existing condition limitations will apply.

  • Medical Examinations An employee may be required by the Employer, at the request of and at the expense of the Employer, to take a medical examination by a physician of the employee's choice. Employees may be required to take skin tests, x-ray examination, vaccination, inoculation and other immunization (with the exception of a rubella vaccination when the employee is of the opinion that a pregnancy is possible), unless the employee's physician has advised in writing that such a procedure may have an adverse affect on the employee's health.

  • Hospital Services The Hospital will: 6.1.1 achieve the Performance Standards described in the Schedules and the HSAA Indicator Technical Specifications; 6.1.2 not reduce, stop, start, expand, cease to provide or transfer the provision of Hospital Services to another hospital or to another site of the Hospital if such action would result in the Hospital being unable to achieve the Performance Standards described in the Schedules and the HSAA Indicator Technical Specifications; and 6.1.3 not restrict or refuse the provision of Hospital Services that are funded by the Funder to an individual, directly or indirectly, based on the geographic area in which the person resides in Ontario, and will establish a policy prohibiting any health care professional providing services at the Hospital, including physicians, from doing the same.
