Medical. There shall be an open enrollment period for medical coverage in each year of this Agreement. An employee may elect no medical coverage during any open enrollment period. An employee who has elected no medical coverage may elect medical coverage during an open enrollment period. No pre-existing condition limitations will apply.
Medical. If Member is medically unable to use the CRUNCH facilities, Member may request a freeze of his or her membership in monthly increments for a minimum of 1 month and a maximum of 6 months per year. Members must provide a doctor’s note at the time of requesting a freeze. CRUNCH reserves the right to verify the note with the doctor and Member agrees to waive any Doctor/Patient privilege and execute any release required by such doctor solely with regards to the medical reasons stated in the doctor’s note for the medical freeze.
Medical. Commencing 7/1/1999, the District will make available to each member the cost of the member’s premium for the member’s HMO plan. If for some reason a member does not wish to enroll in a District- sponsored medical plan, the member may elect to receive $200 for the year in lieu of medical insurance to be applied to other insurance benefits or the Section 125 IRS plan, as approved by the District.
Medical. 1. The Board will pay 80% of the premium and the employee will pay 20% for full-time employees.
Medical. Current health insurance coverage is required for every student. Students are eligible to enroll in a University health policy if they are not covered. Enrollment forms are available through the Business Office. Students must provide proof of health insurance prior to being assigned housing. Students will be automatically billed for health insurance unless the online waiver is completed.
Medical. Upon the request of a permanent employee who has exhausted all accrued sick leave, a leave of absence without pay shall be granted by the Appointing Authority for up to one (1) year because of sickness or injury to the employee. At the request of the employee, this leave may be extended at the discretion of the Appointing Authority. An employee requesting a medical leave of absence shall be required to furnish evidence of disability to the Appointing Authority. When the Appointing Authority has evidence that an employee's absence from duty is unnecessary or if the employee fails to undergo an evaluation or furnish such reports as are required by the Appointing Authority, the Appointing Authority shall have the right to require the employee to return to work on a specified date.
Medical. Employee and any dependents shall be provided Horizon Blue Cross / Blue Shield of New Jersey hospital, medical-surgical and major medical benefits. Employee is not eligible for traditional indemnity coverage, but may select any other plan available in the district, and may select, family, member/spouse, member/child or single coverage.
Medical. 12.1 You acknowledge it is an essential requirement of this Agreement that the NZOC is kept fully informed if you are suffering any physical or psychological injury, illness, condition or impairment that might prevent you from preparing for, or competing in, the Games to the highest possible standard. As such, you agree:
(a) to ensure the NZOC is aware of any illness, injury or condition that may prevent you preparing for, or competing in, the Games to the highest possible standard as soon as you are aware of it;
(b) to arrange and undertake (at such cost agreed with the NZOC), health examinations and medical screens as may reasonably be required by the NZOC and/or the NZOC’s Games appointed medical personnel, to ensure:
i. optimal health strategies are able to be facilitated for the Games;
ii. the risk of an inadvertent anti-doping violation through the use of medications that may require a therapeutic use exemption is minimised; and
iii. your ability or otherwise to compete at the Games to the highest possible standard is able to be determined. This will, at a minimum, include completing a pre-Games health evaluation in accordance with the NZOC Tokyo 2020 Medical Guidelines as prescribed by the NZOC, which will, at a minimum, include undergoing an electrocardiogram (“ECG”), and following the vaccination schedule guidelines for the Games;
(c) any information about you obtained from your disclosure and/or the health examination(s) in sub-clauses 12.1(a) and (b), shall be made available to the NZOC’s Games appointed medical personnel. On the termination or expiry of this Agreement, such health information will be securely stored in line with the Medical Council of New Zealand requirements for the management of medical records, unless you advise the NZOC otherwise, in writing. You have rights in relation to this information under the Privacy Act and as further specified in this Agreement;
(d) the NZOC will arrange for the results of any health examinations and medical screens to be interpreted and assessed by the NZOC’s Games appointed medical personnel. You will be contacted directly by the NZOC’s Games appointed medical personnel where follow-up is required and you agree to undertake, at your own cost, any follow-up procedures, the results of which shall be made available to the NZOC’s Games appointed medical personnel;
(e) that any information obtained about you, that relates to your fitness or otherwise to prepare for or compete in the Games to the highe...
