Mobile Intensive Care Nurse Certification Differential. Registered nurses assigned to the Arrowhead Regional Medical Center (ARMC) and the Emergency Medical Services Nurses assigned to the Inland Counties Emergency Management Agency (ICEMA) who are required and approved by their Appointing Authority to maintain a certificate as a Mobile Intensive Care Nurse (MICN) shall be entitled to a differential of three dollars ($3.00) per hour above their base hourly rate of pay for all hours actually worked. In order to be eligible for the MICN Differential, registered nurses at ARMC shall complete two full shifts per month, as assigned, actually performing the MICN function and perform 10 critiques of radio runs assigned by the nurse supervisor per shift worked. Additionally, there shall be a quarterly list created of a minimum of three (3) day shift and three (3) night shift MICN certified nurses. The purpose of the list is that in the event that there is an open shift for which no MICN certified nurse volunteers to cover, a nurse from the list shall be assigned to that shift. Such assignment shall be made starting with the nurse with the least seniority.

ARMC shall determine the number of nurses required to maintain the MICN certificate. In order to be eligible for the MICN Differential, registered nurses at ARMC shall sign-in and complete two full shifts per 4- week schedule, as assigned, actually performing the MICN function and perform 10 critiques of radio runs assigned by the nurse supervisor per shift worked. The two required shifts performing the MICN function shall generally be extra shifts to the nurses regular work schedule; provided, however, that ARMC may approve the MICN shifts as part of the employee’s regular work schedule. If the MICN shift is an extra shift, the employee shall not be permitted to code leave time during that shift. If the MICN shift is part of the employees regular work schedule, any leave time coded shall not count toward “completion” of the MICN shift and the employee shall be required to work a sufficient number of shifts or hours to satisfy the requirement. If during a scheduled MICN shift a nurse is reassigned to the floor, all hours of that reassignment shall count towards the required MICN hours. There shall be a quarterly list created of a minimum of three (3) day shift and three (3) night shift MICN certified nurses. The purpose of the list is that in the event that there is an open shift for which no MICN certified nurse volunteers to cover, a nurse from the list shall be assigned to that shift. A “shift” shall mean a twelve-hour shift or an equivalent number of hours. Such assignment shall be made starting with the nurse with the least seniority. A nurse who fails to work a sufficient number of shifts or hours to satisfy this requirement during the 4-week schedule shall only be paid the MICN differential for the hours actually worked as a MICN during that 4-week schedule. A full-time MICN shall be permitted to take up to 4-week vacation period without being removed as a full-time MICN; however, the nurse shall not receive the MICN differential during such vacation period.   • Certification Differential Nurses who obtain and maintain a nationally 17 recognized certification that is applicable to the services they provide for the 18 Employer shall receive $1.00 per hour. No additional differential is allotted for 19 more than one (1) certification. Proof of certification must be provided to the 20 Employer upon request. The approved certification list established by mutual 21 consent between the Association and the Director of Human Resources is as 22 follows:

CONTRACTOR shall require such employees, agents, subcontractors, and all other individuals performing services under this Agreement to sign a statement acknowledging the child abuse reporting requirements set forth in Sections 11166 and 11166.05 of the Penal Code and the dependent adult and elder abuse reporting requirements, as set forth in Section 15630 of the WIC, and shall comply with the provisions of these code sections, as they now exist or as they may hereafter be amended.

  • Bilingual Differential When formally assigned in the employee’s position description, an employee assigned to interpret to or from another language to English will receive a differential of five percent (5%) of base pay.

  • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate.   • Preventive Care and Early Detection Services This plan covers, early detection services, preventive care services, and immunizations or vaccinations in accordance with state and federal law, including the Affordable Care Act (ACA), as set forth below and in accordance with the guidelines of the following resources: • services that have an A or B rating in the current recommendations of the U.S. Preventative Services Task Force (USPSTF); • immunizations recommended by the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention; • preventive care and screenings for infants, children, and adolescents as outlined in the comprehensive guidelines supported by the Health Resources and Services Administration (HRSA); or • preventive care and screenings for women as outlined in the comprehensive guidelines as supported by HRSA. Covered early detection services, preventive care services and adult and pediatric immunizations or vaccinations are based on the most currently available guidelines and are subject to change. The amount you pay for preventive services will be different from the amount you pay for diagnostic procedures and non-preventive services.   • Therapist Availability Therapist’s office is equipped with a confidential voice mail system that allows Patient to leave a message at any time. Therapist will make every effort to return calls within 24 hours (or by the next business day), but cannot guarantee the calls will be returned immediately. Therapist is unable to provide 24-hour crisis service. In the event that Patient is feeling unsafe or requires immediate medical or psychiatric assistance, he/she should call 911, or go to the nearest emergency room.

  • Contractor Sales Reporting Vendor Management Fee Contractor Reports Master Contract Sales Reporting. Contractor shall report total Master Contract sales quarterly to Enterprise Services, as set forth below. Master Contract Sales Reporting System. Contractor shall report quarterly Master Contract sales in Enterprise Services’ Master Contract Sales Reporting System. Enterprise Services will provide Contractor with a login password and a vendor number. The password and vendor number will be provided to the Sales Reporting Representative(s) listed on Contractor’s Bidder Profile. Data. Each sales report must identify every authorized Purchaser by name as it is known to Enterprise Services and its total combined sales amount invoiced during the reporting period (i.e., sales of an entire agency or political subdivision, not its individual subsections). The “Miscellaneous” option may be used only with prior approval by Enterprise Services. Upon request, Contractor shall provide contact information for all authorized purchasers specified herein during the term of the Master Contract. If there are no Master Contract sales during the reporting period, Contractor must report zero sales. Due dates for Master Contract Sales Reporting. Contractor shall pay to Enterprise Services a vendor management fee (“VMF”) of 0.74 percent on the purchase price for all Master Contract sales (the purchase price is the total invoice price less applicable sales tax). The sum owed by Contractor to Enterprise Services as a result of the VMF is calculated as follows: Amount owed to Enterprise Services = Total Master Contract sales invoiced (not including sales tax) x .0074. The VMF must be rolled into Contractor’s current pricing. The VMF must not be shown as a separate line item on any invoice unless specifically requested and approved by Enterprise Services. Enterprise Services will invoice Contractor quarterly based on Master Contract sales reported by Contractor. Contractors are not to remit payment until they receive an invoice from Enterprise Services. Contractor’s VMF payment to Enterprise Services must reference this Master Contract number, work request number (if applicable), the year and quarter for which the VMF is being remitted, and the Contractor’s name as set forth in this Master Contract, if not already included on the face of the check. Failure to accurately report total net sales, to submit a timely usage report, or remit timely payment of the VMF, may be cause for Master Contract termination or the exercise of other remedies provided by law. Without limiting any other available remedies, the Parties agree that Contractor’s failure to remit to Enterprise Services timely payment of the VMF shall obligate Contractor to pay to Enterprise Services, to offset the administrative and transaction costs incurred by the State to identify, process, and collect such sums. the sum of $200.00 or twenty-five percent (25%) of the outstanding amount, whichever is greater, or the maximum allowed by law, if less. Enterprise Services reserves the right, upon thirty (30) days advance written notice, to increase, reduce, or eliminate the VMF for subsequent purchases, and reserves the right to renegotiate Master Contract pricing with Contractor when any subsequent adjustment of the VMF might justify a change in pricing.

  • Physician Visits This plan covers the services of a physician or other provider in charge of your medical care while you are inpatient in a general or specialty hospital.

  • REPORT ON CONTRACT SALES ACTIVITY AND ADMINISTRATIVE FEE PAYMENT A. CONTRACT SALES ACTIVITY REPORT. Each calendar quarter, Supplier must provide a contract sales activity report (Report) to the Sourcewell Supplier Development Administrator assigned to this Contract. Reports are due no later than 45 days after the end of each calendar quarter. A Report must be provided regardless of the number or amount of sales during that quarter (i.e., if there are no sales, Supplier must submit a report indicating no sales were made). The Report must contain the following fields: • Participating Entity Name (e.g., City of Staples Highway Department); • Participating Entity Physical Street Address; • Participating Entity City; • Participating Entity State/Province; • Participating Entity Zip/Postal Code; • Participating Entity Contact Name; • Participating Entity Contact Email Address; • Participating Entity Contact Telephone Number; • Sourcewell Assigned Entity/Participating Entity Number; • Item Purchased Description; • Item Purchased Price; • Sourcewell Administrative Fee Applied; and • Date Purchase was invoiced/sale was recognized as revenue by Supplier.
