Monthly Premium Payments. Premium payments are due in advance of the month covered. Monthly premium payments must be made to KFHPWA in compliance with KFHPWA’s terms and guidelines:
a. Off-Exchange (Direct) • Initial payment will be billed once enrollment has been completed • Correct premium amount must be received on or before the first of the month. • Payments are accepted via: (i) online at xxx.xx.xxx/xx/xxxxxxxxx; (ii) phone at 000-000-0000; or,

Premium rates will be revised as a part of the annual renewal process. KFHPWA reserves the right to re-rate this benefit package if the demographic characteristics change by more than 15%. KFHPWA will establish a Rate Stabilization Reserve (RSR) for the Group that is based upon the financial performance of this Group Agreement and all other Group Agreements in effect between KFHPWA and Xxxxxx Foundation Health Plan of Washington Options, Inc. (“KFHPWAO”) and the Group. In the event that KFHPWA and KFHPWAO both issue plans that are subject to the RSR provision, the KFHPWA shall administer the RSR and the parties to this Agreement that the terms and conditions set forth in Group’s Agreement with KFHPWA regarding the RSR will control.

Payment must be received on or before the due date and is subject to a grace period of ten (10) days. Premiums are subject to change by GHC upon thirty (30) days written notice. Premium rates will be revised as a part of the annual renewal process.
Monthly Premium Payments. For the initial term of this Agreement, the Group shall submit to Group Health for each Member the monthly premiums set forth in the current Premium Schedule and a verification of enrollment. Payment must be received on or before the due date and is subject to a grace period of 10 days. Premiums are subject to change by Group Health upon 30 days written notice. Premium rates will be revised as a part of the annual renewal process. Group Health reserves the right to re-rate this benefit package if the demographic characteristics change by more than 15%.
Monthly Premium Payments. (a) CONTRACTOR agrees to provide the Services and Deliverables described in this Agreement for monthly premium payments to be paid by HHSC to CONTRACTOR.
(b) CONTRACTOR understands and expressly assumes the risks associated with the performance of the duties and responsibilities under this Agreement, including the failure, termination or suspension of funding to HHSC, delays or denials of required approvals, and cost overruns not reasonably attributable to HHSC. To the extent that funding or required approvals are not provided, CONTRACTOR is not further obligated to provide Services or Deliverables beyond any Service or Deliverable for which HHSC can provide acceptable assurances of available funding.
(c) CONTRACTOR further agrees that:
(1) No additional charges, fees, or costs will be added to the monthly premium amount and the delivery supplemental payment described in section 10.03 or sought except for properly authorized and executed Change Orders; and
(2) No other charges for tasks, functions, or activities that are incidental or ancillary to the delivery of the Services and Deliverables will be sought from HHSC or any other state agency, nor will the failure of HHSC or any other party to pay for such incidental or ancillary services entitle CONTRACTOR to withhold Services or Deliverables due under the Agreement.
(d) A CONTRACTOR's monthly premium payment will not be reduced for a family's failure to make its premium payment. There is no relationship between the per member/per month amount owed to an CONTRACTOR for coverage provided during a month and the family's payment of its premium obligation for that month.

Monthly Premium Payments. For the initial term of this Agreement, the Group shall submit to KFHPWA for each Member the monthly premiums set forth in the current Premium Schedule and a verification of enrollment. Payment must be received on or before the due date and is subject to a grace period of 10 days. Premiums are subject to change by KFHPWA upon 30 days written notice. Premium rates will be revised as a part of the annual renewal process. KFHPWA reserves the right to re-rate this benefit package if the demographic characteristics change by more than 15%. Surviving Dependents of a Subscriber will remain covered for up to 12 months without payment of premium provided all eligibility requirements under the agreement are met. This continued coverage will apply only in the event the Dependent(s) were enrolled under the agreement at the time of the Subscriber’s death. This 12 month period will apply toward the COBRA continuation period, if elected, and will not extend any other continuation benefits the Group may offer. Surviving Dependents who become ineligible for coverage during the 12-month extension of benefits following the Subscriber’s death may apply for COBRA and the premium waiver will no longer apply.
Monthly Premium Payments. CONTRACTOR will be required to provide in a timely manner financial and statistical information necessary in the capitation rate determination process. Encounter data provided by CONTRACTOR must conform to all HHSC requirements. Encounter data containing non-compliant information, including, but not limited to, inaccurate member identification numbers, inaccurate provider identification numbers, or diagnosis or procedure codes insufficient to adequately describe the diagnosis or medical procedure performed, will not be considered in the CONTRACTOR'S experience for rate-setting purposes. Information or data, including complete and accurate encounter data, as requested by HHSC for rate-setting purposes, must be provided by CONTRACTOR to HHSC: (1) within thirty (30) days of receipt of the letter from HHSC requesting the information or data; and (2) no later than March 31st of each year.
