Orthodontia Coverage. The County will continue to offer Orthodontia coverage to all permanent employees in classes represented by the DSA who participate in a County dental plan. The cost for orthodontia coverage is borne 100% by each enrolled employee. Premium payments are made by payroll deduction or direct pay, as applicable.

Orthodontia Coverage. Orthodontia coverage will be provided for each EMPLOYEE and each dependent at 50% of charges to a maximum of $1500 per person per lifetime.
Orthodontia Coverage. After an employee has completed one full year of service as a regular full-time employee, he or she will become eligible for orthodontia benefits. If an employee or dependent requires orthodontia care, the Employer agrees to pay fifty percent (50%) of the charges up to an aggregate $1,500 lifetime maximum benefit per person.

  • Medical Coverage The Executive shall be entitled to such continuation of health care coverage as is required under, and in accordance with, applicable law or otherwise provided in accordance with the Company’s policies. The Executive shall be notified in writing of the Executive’s rights to continue such coverage after the termination of the Executive’s employment pursuant to this Section 3(d)(iv), provided that the Executive timely complies with the conditions to continue such coverage. The Executive understands and acknowledges that the Executive is responsible to make all payments required for any such continued health care coverage that the Executive may choose to receive.

The Company shall have no further obligations to the Executive as a result of termination of employment described in this Section 8(a) except as set forth in Section 12.
