ORTHODONTIC BENEFITS. Orthodontic Benefits are provided only for qualified dependent children less than nineteen (19) on the date the orthodontic procedure commences. A separate lifetime deductible of $50 per individual applies to orthodontic treatment. After the deductible has been satisfied, the plan will pay 70% of the orthodontist's actual charges up to the plan maximum of $1,500.
ORTHODONTIC BENEFITS. Delta Dental will provide Orthodontic Benefits in accordance with the Orthodontic Benefit Rider attached hereto as Appendix A.

ORTHODONTIC BENEFITS. Delta agrees to provide Orthodontic Benefits in accordance with the Orthodontic Benefit Rider attached hereto as Appendix C.
ORTHODONTIC BENEFITS. The prevention or correction of irregularities in the alignment of the teeth and the prevention or correction of the malocclusion, as follows: The correction of dysfunctional malocclusion consisting of the following:
1. Diagnosis, including models and radiographs;
2. Active treatment, including necessary appliances; and,
3. Retention treatment following active treatment, limited to ten (10) visits in an eighteen (18) month period.
ORTHODONTIC BENEFITS. Orthodontic Benefits are subject to a $1,500.00 maximum per case which is separate from the $1,500.00 maximum mentioned above applicable to Basic and Prosthodontic Benefits. ORTHODONTIC BENEFITS. The District shall provide orthodontic coverage to all MBUs, spouses and dependents covered by the District’s dental plan. The plan shall cover 100% of expenses. A lifetime maximum of $1000 per covered individual shall apply.

ORTHODONTIC BENEFITS. The District shall provide for orthodontic benefits and coverage for all eligible employees and dependents. Such coverage shall be 50/50 coinsurance with a three thousand dollar ($3000) lifetime maximum per patient with no deductible. Also, the Union agrees that any increase in orthodontic premiums will be paid for by each individual employee during the term of this Agreement notwithstanding any other language. Newly-hired employees will have a one-year waiting period, without claims, for orthodontic coverage.
