Pay-Per-Use. The other new aspect of cloud computing is application of usage-based billing model. Users simply pay for the services they get while providers bear the costs of hardware and software provision. Pricing may vary depending on the time of day due to peaks in demand or varying electricity costs and institutions may therefore carry out certain activities when costs are cheaper. Pricing is the process of determining what a service provider will receive from an end user in exchange for their services (X.Xxxxxxxxx, [11]) claimed that cloud computing success in the IT market can be obtained only by developing adequate pricing techniques. The pricing process can be as follows: fixed, in which the customer is charged the same amount all the time; dynamic, in which the price charged changes dynamically; or market-dependent, in which the customer is charged based on the real-time market conditions. Fixed pricing mechanisms include the pay- per-use model, in which the customers pay for the amount they consume of a product or the amount of time they use a certain service.
Pay-Per-Use. 4B.1 e-LATiH grants a lifetime access license to the Learner when they enroll in a Pay-per-use course. If a prescription drug is not on our formulary, it is not covered. Prescription drugs and diabetic equipment or supplies are covered when dispensed using the following guidelines: • the prescription must be medically necessary, consistent with the physician’s diagnosis, ordered by a physician whose license allows him or her to order it, filled at a pharmacy whose license allows such a prescription to be filled, and filled according to state and federal laws; • the prescription must consist of legend drugs that require a physician’s prescription under law, or compound medications made up of at least one legend drug requiring a physician’s prescription under law; • the prescription must be dispensed at the proper place of service as determined by our Pharmacy and Therapeutics Committee. For example, certain prescription drugs may only be covered when obtained from a specialty pharmacy; and • the prescription is limited to the quantities authorized by your physician not to exceed the quantity listed in the Summary of Pharmacy Benefits. Prescription drugs are subject to the benefit limits and the amount you pay shown in the Summary of Pharmacy Benefits.

  • Leave Without Pay for the Care and Nurturing of Pre-School Age Children Subject to operational requirements, an employee shall be granted leave without pay for the personal care and nurturing of the employee's pre-school age children in accordance with the following conditions:
