Performance Reporting Requirements. Grantees must have the capability to report projected activities, participant, and outcome data to the state, in a manner that is timely, thorough, and accurate. When applicable, grantees will be required to use the State’s system for reporting participant data collected. If applicable, the Grantee will be required to have access to the state’s CalJOBSSM reporting system. The State will provide training on how to use the CalJOBS reporting system. Additionally, Grantees will be required to submit monthly progress status reports which include narrative on the status of the projects. A. A team of program support will be assigned after the signing of this contract, at which time the EDD will provide all contact information. B. Schedule monthly calls with the EDD, OPR, and GO-Biz to discuss: • Project updates: status and goals • Sustainable goals for the organization • Evaluating and assessing the programs' strengths and weaknesses.

Performance Reporting Requirements. Contractor shall provide City with reports and records related to the performance of services under this Contract. City reserves the right to final review and approval of the format on all reports, to request additional reports and/or request changes to existing reports, during the term of this Contract at no additional cost. Contractor-generated reports shall include, but are not limited to: • Utilization review status • Xxxx review status • PPO network(s) penetration and savings below Fee Schedule • Duplicate bills reviewed • Aging report • Xxxx review turnaround time • Savings Contractor shall provide ad hoc reports upon request within a mutually agreed upon timeframe at no cost to City. Contractor shall provide a comprehensive annual statistical summary and narrative report evaluating City’s cost containment program documenting savings and making recommendations for improvement

In addition, the Contractor shall describe the efforts it undertook to prevent use or submission of covered telecommunications equipment or services, and any additional efforts that will be incorporated to prevent future use or submission of covered telecommunications equipment or services.
