The Permanent Supportive Housing clause establishes the framework for providing long-term housing assistance combined with supportive services to individuals or families experiencing chronic homelessness or with special needs. Typically, this clause outlines eligibility criteria, the nature of housing provided (such as non-time-limited rental units), and the types of support services available, which may include case management, healthcare access, or employment assistance. Its core function is to ensure stable housing for vulnerable populations while addressing underlying issues that contribute to homelessness, thereby promoting housing stability and reducing the risk of returning to homelessness.
Permanent Supportive Housing. The Project shall provide [ ] ([ ]) units complying with Permanent Supportive Housing requirements under the QAP for the term provided therein. With respect to such units, Owner has executed and delivered a Permanent Supportive Housing Set-Aside Agreement acceptable to LHC and will accept renewals of such agreements on the same or otherwise mutually agreeable terms. Owner agrees that vouchers will only be awarded for [ [ ([ ]) of the units.
Permanent Supportive Housing. For the purpose of this ITN, housing paired with community-based services with onsite case management and/or service coordination to assist Persons with Special Needs or Homeless households achieve housing stability that allows an indefinite length of stay as long as the tenant complies with lease requirements and has a lease with a minimum of seven months with no requirements related to the provision of or participation in supportive services. Permanent Supportive Housing shall facilitate and promote activities of daily living, access to community-based services, and inclusion in the general community. It is possible that Permanent Supportive Housing units may be embedded in a broader Affordable Housing property.
Permanent Supportive Housing. “Permanent Supportive Housing” means permanent housing with supportive services to assist People Experiencing Homelessness who have a disability to live independently. Supportive services are designed to meet the needs of participants and must be offered for the entire duration of program participation. Permanent Supportive Housing may be single site, scattered site, or clustered, and can be integrated with affordable or market-rate units. Housing assistance can be project-based (tied to a unit) or tenant-based (tenant must secure a unit in the market).

After HAP Agreement has been approved by the City, Mercy House will work with City Homeless Solutions and City Net to establish a list of eligible Corona homeless tenants, collaborate with City Net to get clients document ready for referral to the County’s Coordinated Entry System and subsequently referred to the Riverside County Housing Authority for approval of a project-based voucher for the 5th Street units. Permanent Supportive Housing. Non-time limited affordable housing for a household that is homeless on entry, and has a condition or disability, such as mental illness, substance use disorder, chronic health issues, or other conditions that create multiple and serious ongoing barriers to housing stability. Households need a long-term high level of services in order to meet the obligations of tenancy and maintain their housing. Tenant holds a rental agreement or lease and may continue tenancy as long as rent is paid and the tenant complies with the rental agreement or lease. Tenants have access to a flexible array of comprehensive services, mostly on site, such as medical and wellness, mental health, substance use disorder, vocational/employment, and life skills. Services are available and encouraged but are not to be required as a condition of tenancy. There is ongoing communication and coordination between supportive service providers, property owners or managers, and/or housing subsidy programs.
Permanent Supportive Housing. 2.7.15.1 The Contractor shall support the Permanent Supportive Housing program, which is a partnership between LDH and the Louisiana Housing Authority (LHA) to provide deeply affordable, community-integrated housing paired with tenancy supports that assist high-risk persons with disabilities to be successful tenants and maintain stable housing. 2.7.15.2 For the Louisiana PSH program, the Contractor shall: 2.7.15.2.1 Assist potentially eligible enrollees in completing the PSH program application; 2.7.15.2.2 Within one (1) business day of request by designated LDH PSH program staff, provide accurate information about current and past service authorizations and encounters, particularly for behavioral health services such as Community Psychiatric Support and Treatment (CPST), Psychosocial Rehabilitation (PSR), and Assertive Community Treatment (ACT); 2.7.15.2.3 Ensure timely prior authorization for PSH tenancy and pre-tenancy supports as applicable; 2.7.15.2.4 Ensure PSH tenancy supports are delivered in a timely and effective manner in accordance with an appropriate plan of care; 2.7.15.2.5 Respond timely to service problems identified by PSH program management, including but not limited to those that place an enrollee’s/tenant’s housing or PSH services at risk; and 2.7.15.2.6 Work with PSH program management to ensure an optimal network of qualified service providers trained by the LDH PSH program staff or designee to provide tenancy supports across disability groups and certified to deliver services as defined in the PSH Provider Certification Requirements. 2.7.15.3 To ensure effective accomplishment of the responsibilities required in this section, the Contractor shall: 2.7.15.3.1 Identify a PSH program liaison, subject to approval by LDH, to work with LDH PSH program staff to ensure effective performance of contract responsibilities and requirements, effective implementation and delivery of PSH services, and to address problems or issues that may arise; 2.7.15.3.2 Assist with statewide targeted outreach to enrollees/households who could benefit from PSH, including those enrollees least likely to apply. Permanent Supportive Housing a. The Joint Office and PHB will each play a role in providing and maintaining Permanent Supportive Housing services. b. PHB will retain any City funding and responsibility for Permanent Supportive Housing that is or will be project-based within its affordable housing portfolio. c. The Joint Office will provide policy advice and may, at PHB’s request, provide contract management services to PHB for Permanent Supportive Housing services.
