Pharmacy Staff. 3.5.1. Pharmacists may delegate the responsibility for supervision of methadone, buprenorphine or naltrexone to appropriately trained staff. However, the pharmacist will remain accountable for supervised consumption and must always be in a position to intervene immediately where necessary. Staff undertaking supervision should be trained pharmacy technicians or healthcare assistants who have also undergone local training in pharmaceutical harm reduction. As a minimum requirement, any staff member involved in supervised consumption must be over 18 years of age and have completed medicine counter assistant training.

Pharmacy Staff. 4.6.1 ‘The pharmacist who is responsible for the service’ may nominate appropriately trained pharmacy staff to co-ordinate the day to day running of the needle exchange, such as ordering stock, conducting needle exchange transactions and recording needle exchange activity. ‘The pharmacist who is responsible for the service’ will ensure that pharmacy staff training criteria are in accordance with item 4.6 below. All Delegated Pharmacy Staff involved in the needle exchange scheme must be adequately trained to do so and named on the pharmacy’s SLA.
Pharmacy Staff. Pharmacist means a person who is registered as a practicing pharmacist under the Pharmacy Act 1964 and with the Pharmacy Board of Australia/AHPRA. Pharmacy Assistant Grade 1 - means a Caregiver appointed as such who is engaged in drug distribution duties, hospital pharmacy production and dispensing activities under the supervision of a Registered Pharmacist and/or Pharmacy Technician. Pharmacy Assistant Grade 2 - means a Caregiver appointed as such who is engaged in drug distribution duties, hospital pharmacy production and dispensing activities under the supervision of a Registered Pharmacist and/or Pharmacy Technician, and who holds a qualification in a relevant field recognised by the Pharmaceutical Society of Australia or up to the level of Certificate III in Community Pharmacy issued by a Registered Training Organisation or has qualifications deemed by the Employer to be equivalent. Pharmacy Technician Grade 1 - means a Caregiver appointed to such a position and who has successfully completed a qualification in a relevant field recognised by the Pharmaceutical Society of Australia or up to the level of Certificate III issued by a Registered Training Organisation in Hospital and Community Pharmacy or has qualifications deemed by the Employer to be equivalent. Pharmacy Technician-Grade 2 - means a Caregiver who is appointed to such a position and who has successfully completed a nationally recognised Pharmacy Technician Certificate Course at Certificate Level IV or has qualifications deemed by the Employer to be equivalent. A Caregiver at this grade under the supervision of a Pharmacist and/or a more senior Pharmacy Technician.

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Supplier Staff The Supplier Staff shall at all times during the Contract Period; obey all lawful instructions and reasonable directions of the Customer; apply all due skill, care, diligence and shall be appropriately experienced, qualified and trained to supply the Services in accordance with this Contract; and shall respond to any enquiries from the Product Owner relating to the Services within the timescales agreed by the Parties (where applicable) and in any event as soon as is reasonably practicable. The Supplier acknowledges and agrees that the continuity of the Key Personnel is paramount to the success of the Project and shall ensure that Key Personnel are assigned to the Project on a full-time basis and are not removed from the Services or assigned to any other Supplier or third party projects during the Contract Period (unless otherwise Approved by the Customer). The Customer may also require the Supplier to remove and/or replace any Key Personnel that the Customer acting reasonably considers in any respect unsatisfactory. The Customer shall not be liable for the cost of removing or replacing any Key Personnel. The Customer may, by written notice to the Supplier, refuse to admit onto, or withdraw permission to remain on, the Customer’s Premises any member of the Supplier Staff or any person employed or engaged by any member of the Supplier Staff whom the Customer believes represents a security risk or does not have the required levels of training and expertise or whose admission or continued presence would, in the reasonable opinion of the Customer, be undesirable. Supplier Staff engaged within the boundaries of the Customer’s Premises shall comply with such rules, regulations and requirements as may be in force from time to time for the conduct of personnel when at or within the boundaries of those Customer’s Premises.

  • Prosthodontics We Cover prosthodontic services as follows: • Removable complete or partial dentures, for Members 15 years of age and above, including six (6) months follow-up care; • Additional services including insertion of identification slips, repairs, relines and rebases and treatment of cleft palate; and • Interim prosthesis for Members five (5) to 15 years of age. We do not Cover implants or implant related services. Fixed bridges are not Covered unless they are required: • For replacement of a single upper anterior (central/lateral incisor or cuspid) in a patient with an otherwise full complement of natural, functional and/or restored teeth; • For cleft palate stabilization; or • Due to the presence of any neurologic or physiologic condition that would preclude the placement of a removable prosthesis, as demonstrated by medical documentation.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias. Procedures include but are not limited to: • Rapid Palatal Expansion (RPE); • Placement of component parts (e.g. brackets, bands); • Interceptive orthodontic treatment; • Comprehensive orthodontic treatment (during which orthodontic appliances are placed for active treatment and periodically adjusted); • Removable appliance therapy; and • Orthodontic retention (removal of appliances, construction and placement of retainers).
