Pre-Service. Supplier will provide leadership, management oversight, staffing and technical expertise of: • Authorization/Referral (Verification) – Supplier will (i) perform authorization clearance services for patients to verify that an authorization is in place, and (ii) document all required authorizations. • Insurance Eligibility Verification (Financial Clearance) – Once the payer is identified, check benefit eligibility and obtain verification from insurance (governmental or commercial) that the patient reported insurance for the applicable service is still in force and will reimburse the provider for the service.
Pre-Service. A request for Precertification or Predetermination that is conducted before the service, treatment or admission.

Add the following definition: CAPITAL EQUIPMENT - Item(s) acquired by CSC with monies from the Welfare and Benefits Fund with a unit cost of $5,000 or more and a useful life of at least one year. Unit cost includes applicable sales tax, freight and other ancillary costs to place the asset in its intended location.
Pre-Service. Before any driver begins transporting students during the time period covered by this contract, Vendor shall, at its sole expense, arrange for a comprehensive drug and alcohol assessment of the driver. The result of this drug and alcohol assessment shall be transmitted directly from the laboratory conducting the testing to CHCCS at least ten (10) days prior to the first day the driver transports students under this Agreement.

i. If your request for benefits is filed improperly, MedTrak must notify you within 5 days.
ii. If your request for benefits is incomplete, MedTrak must notify you within 15 days.
iii. You must then provide completed request for benefits information to MedTrak within 45 days.
iv. MedTrak must notify you of the benefit determination within 15 days of a completed request or upon the receipt of all additional required information if your initial request was incomplete.
v. You must appeal an adverse benefit determination no later than 180 days after receiving such determination.
vi. MedTrak must notify you of the internal appeal decision within 15 days of receiving such appeal.
Pre-Service. Claim means a request for a Claims decision when prior authorization of the services is required by the Company.
