Private Duty Nurse. When approved in advance by MSH Assistance and prescribed by an attending physician, the professional services of a registered private duty nurse (other than an immediate family member) as the result of a covered Pandemic related emergency when medically necessary and while hospitalized or in lieu of hospitalization.

Private Duty Nurse. The professional services of a registered private nurse. Emergency Air Transportation: Air ambulance to the nearest appropriate medical facility or to a Canadian hospital for immediate emergency medical treatment including transport on a licensed airline with an attendant, when required. Medical Evacuation: Subject to the discretion of CanAssistance, medical evacuation by air ambulance, or transport on a licensed airline with an attendant when required, to Canada for continued immediate medical treatment. Emergency Remote Evacuation: When a regular ambulance service cannot be used, emergency evacuation from a mountain, body of water or other remote location by a commercial operator licensed to convey passengers to the nearest qualified medical facility capable of providing appropriate treatment, to a maximum payment of $5,000. Transportation to Bedside: Cost of a single round-trip economy airfare from Canada, plus up to $150 per day for the cost of meals and commercial accommodation for one of the following immediate family members or a close personal friend, to be with you if you are travelling alone and hospitalized as the result of an emergency. To be payable, this benefit requires that you be hospitalized as an inpatient for more than three (3) consecutive days outside your province or territory of residence and that the attending physician provide written certification that your medical condition was serious enough to warrant the visit; or where legally necessary, identify your remains prior to their release.

Before starting work for you, the person you hire must contact the Director of Nursing Services to discuss the guidelines which govern private duty nurses. We do not have a private duty nursing registry at Xxxxxxxx House, but you may get information on how to obtain a private duty nurse by speaking with our Director of Nursing Services. All financial arrangements will be made directly between you and the private duty nurse. Room Changes Every effort will be made to place you with a compatible roommate and keep room changes to a minimum. Occasionally, however, it may be necessary to transfer a resident from one room to another for medical or administrative reasons, or to better meet individual needs. Xxxxxxxx House reserves the right to make such changes at its discretion. You will be notified of any pending changes as soon as possible.

Any questions concerning the classroom will be directed to the building principal and/or Supervisor of Special Education/Pupil Services. ● The private duty agency nurse must adhere to the established classroom rules and procedures. Classroom instruction and student discipline are the responsibility of the classroom teacher. ● The private duty agency nurse will be responsible for behavior consistent with appropriate professional practice and policies of the Bristol Township School District. ● The private duty agency nurse will function under direct supervision of the respective building principal and/or the Supervisor of Special Education/Pupil Services, in consultation with the outside provider supervisor. ● The private duty agency nurse will use school telephones only in the case of an emergency. Cell phones are permitted in the buildings (unless otherwise specified by the school) but must be turned off during the school day. The private duty agency nurse may use their cell phone while on breaks or outside of the classroom. ● The private duty agency nurse will maintain the least intrusive role in the classroom. When not working directly with an identified student, the private duty agency nurse will quietly observe and monitor the student. ● The private duty agency nurse does not have the authority to take a student out of the building. In the event the student becomes ill during the school day, the parent will be required to arrive at the school and sign the student out in the main office. ● District staff will exchange information with the agency nurse only after the parent/guardian has g...

  • CHILD AND DEPENDENT ADULT/ELDER ABUSE REPORTING CONTRACTOR shall establish a procedure acceptable to ADMINISTRATOR to ensure that all employees, agents, subcontractors, and all other individuals performing services under this Contract report child abuse or neglect to one of the agencies specified in Penal Code Section 11165.9 and dependent adult or elder abuse as defined in Section 15610.07 of the WIC to one of the agencies specified in WIC Section 15630.
