The Contractor’s staff will maintain a log of all contractors who performed routine work on the facility, to include date, time, and work performed. To avoid unauthorized work or duplicate applications for service or project work, DES will accept requests only from designated DHS and program staff. The Contractor will provide DES with names of designated staff who are authorized to submit work requests. As follows:
