Work Requests. Work Requests will be issued by either Party where updates/modifications to Services are requested and will require the execution of Work Orders.
Work Requests. All work requests submitted to the Director shall be in triplicate and contain the bus driver's name, bus number, date, mileage, and a description of the work to be completed.
Work Requests. 19.4.1. All requests for maintenance and repair work will be received by the Assistant Superintendent, Business Services or his/her designee. He/she will deliver them directly to the persons responsible in the Teaching/Learning Center and at the High Point School.
19.4.2. These requests will be initialed by the person doing the work at such time as the work is completed and returned to either the Assistant Superintendent, Business Services or his/her designee.

It shall be the responsibility of the Beneficiary and the Consultant, not the State Center, to resolve any potential conflicts of interest between

Before submitting a Work Request to the State Center, the Beneficiary shall secure the consent of a member of the State Center Panel of Consultants to provide the Consulting Services required. It shall be the responsibility of the Beneficiary and the Consultant, not the State Center, to resolve any potential conflicts of interest between them. The Beneficiary shall deliver a Work Request, describing the Consulting Services to be provided, to the State Center’s Executive Director, with a copy to the Consultant, which shall be substantially in the form of Exhibit 1 (“Work Request”) attached hereto. The State Center’s approval of the Work Request triggers all the rights and obligations of the State Center, the Beneficiary and the Consultant under this Agreement. The Work Request shall be subject to all of the terms and conditions of this Agreement, unless the Work Request specifically includes any additional or different terms and specifically identifies whatever terms in this Agreement that are to be amended by such additional or different term.
Work Requests. The Online Service includes a “Smartsourcing” tool that allows you to submit requests to an on- demand work force for completion of simple tasks (“Work Requests”). If you place a Work Request through the Online Service, Smartsheet uses Xxxxxx.xxx’s “Mechanical Turk” and/or other third party services to submit Work Requests to third parties who may wish to perform such tasks (“Service Providers”).
Work Requests. The Operator shall fulfil any work requests the Client might make (hereafter, Work Request) in accordance with the terms and times established in the relevant section of Annex 1 (Description of Services). It should be noted that the failure to issue a Work Request shall not mean the Operator is exempt from correctly fulfilling the obligations required of it by this Contract. The Client must provide the Operator with all the technical information it has that is required to deal with a Work Request.
Work Requests. Any request by EPI for Trade Dress changes, ------------- packaging changes or changes in Product Support shall be made by filling out the forms set forth in Schedule E and forwarding them to the applicable DMPC Representative. In the event that EPI requests a Trade Dress change, all costs associated with implementation of such change, including the cost of all obsolete labeling components, will be borne by EPI. In the event EPI requests a packaging change, the costs associated with implementation of such change, above the level set forth in Schedule D for Product Support, will be borne by EPI; EPI shall also bear the cost of all obsolete packaging components. The costs associated with Product Support in excess of the levels set forth in Schedule D shall be borne by EPI and shall be determined pursuant to Section 4.3(d).
Work Requests. The requirements of the tenants of the Building will be attended to only upon application at the office of the Landlord. Building service employees shall not perform any work or do anything outside their regular duties, unless under special instruction from the Landlord.
Work Requests. 1. At least two weeks prior to starting work, except in an emergency, the DWR Contract Manager will provide the Contractor with a Traffic Service Request Form, Exhibit A, Attachment 1, including details of the location, scope, and proposed schedule of needed traffic control service. The DWR Contract Manager will provide locations by maps, street address, roadway intersections, GPS coordinates, or any other means accurately identify the locations permitting the Contractor to create plans, determine the materials and labor needed, and accurately apply for necessary permits. A job walk can be requested for any complex projects that it is deemed necessary by the Contractor or the DWR Contract Manager. When work is taking place within 6 feet of the highway, traffic will be controlled in accordance with the current California MUTCD. Adequate signage, approved by DWR and other required agencies, will be erected to advise the public of construction activities in the vicinity. The Traffic Control Plan will allow motorists to pass around the blocked lane with as little delay as possible. Once the Contractor receives the completed Service Request Form, the Contractor will have one week to provide DWR with a detailed Traffic Control Plan including the proposed materials, labor hours, plan costs (with and without a Contractor’s stamp), and estimated time frame for obtaining necessary permits. The DWR Contract Manager will have one week to plan and to notify the Contractor of the approval. Work will begin based on work dates provided by the DWR Contract Manager. DWR will not pay for services that have not been approved. DWR can edit the Traffic Service Request Form at any time.
2. In the case of an emergency, DWR will provide the Contractor with a 12-hour notice to be onsite with a DWR-approved Traffic Control Plan.
3. Itemized invoices must be submitted promptly after work is completed. The itemized invoices must include the original Traffic Service Request Form, and Traffic Control Plan for the DWR Contract Manager’s review.
