Common use of Proof of Illness Clause in Contracts

Proof of Illness. (i) An employee may be required to produce a certificate from a medical practitioner for any illness after five (5) consecutive working days certifying that the employee was unable to carry out the required duties due to illness. A form outlining the required information will be provided by the employer and the employee will have it completed by their physician. (ii) The Board agrees to reimburse the employee for the cost of the medical certificate requested in subsection (i) above.

Proof of Illness. (i) An employee may be required to produce a certificate from a medical practitioner for any illness after five (5) consecutive working days certifying that the employee was unable to carry out the required duties due to illness. (ii) An employee may be provided with a list of medical practitioners by the Board, and shall produce the certificate from one of those practitioners, in cases of chronic absenteeism or where there is a pattern of sick leave that concerns the employer. (iii) The Board agrees to reimburse the employee for the cost of the medical certificate requested in subsection (iii) above.

(ia) An The Employer may request an employee may be required to produce a certificate from a qualified medical practitioner for the third instance of illness in a calendar year and/or any illness after five (5) consecutive working exceeding three days certifying that the such employee was is unable to carry out the required his duties due to illness. A form outlining the required information will be provided by the employer and the employee will have it completed by their physicianillness or non-compensable accident. (iib) The Board agrees to reimburse request for a certificate, when required, will be made at the time that the employee for notifies the Employer that he is ill. (c) The cost of supplying such written information shall be borne by the medical certificate requested in subsection (i) aboveEmployer.

(ia) An employee may be required to produce a certificate from a duly qualified medical practitioner for any illness after five absence in excess of three (53) consecutive working days days, certifying the employee’s absence was due to illness. b) In the case of a prolonged illness, the Employer may require progress reports from time to time from the employee’s physician certifying that the employee was unable to carry out is unfit for work; the required duties due to illness. A form outlining the required information will be provided by the employer and dates the employee will have it completed by their physicianis advised to remain off work; and, if possible, an expected return to work date. The Employer shall be responsible for payment for such information. (ii) The Board agrees to reimburse the employee for the cost of the medical certificate requested in subsection (i) above.
