Proof Required. The Department Head shall approve sick leave only after having ascertained that the absence was for an authorized reason. The Department Head may require the employee to submit substantiating evidence including but not limited to a physician's certificate in accordance with Federal and State law. The request for substantiating evidence must be made either on or before the time the employee informs the department of his/her absence or prior to the employee's return to work. If the Department Head does not consider the evidence adequate, the employee’s request for the use of sick leave shall not be approved.
Proof Required. 6.6.1. In support of any request for an extension of the Contract Time, Construction Manager must demonstrate to the reasonable satisfaction of Owner that the critical path of the Project Schedule was delayed and such change delayed the Date of Substantial Completion. Construction Manager will be entitled to an increase in the Contract Time for the number of days that the Date of Substantial Completion was delayed solely as a result of the compensable or excusable event. Construction Manager will compare the critical path of the Project Schedule to the actual critical path of the Work, identifying the specific impact of the compensable or excusable event.
6.6.2. Construction Manager will submit to the Owner a written time impact analysis illustrating the influence of each compensable or excusable event on the Date of Substantial Completion. Each time impact analysis will include a fragmentary network (network analysis) demonstrating how the Construction Manager proposes to incorporate the time impact based on the date of the delay in time and the event time computation of all affected activities.
Proof Required. Proof of an employee’s need for Xxxxx’s Leave may be established through a number of means, including but not limited to: i Provision of medical certificates from a certified medical practitioner detailing nature of illness of dependant.
Proof Required. The employee must prove to the employer’s satisfaction that the absence from work was caused by illness or injury. If the proof is disputed, it will be dealt with in accordance with clause 11 – Procedure for avoiding industrial disputes of this award.

Proof Required. 36.4.1 Proof of an employee’s need for Carer’s Leave may be established through a number of means, including but not limited to:
i. Provision of medical certificates from a certified medical practitioner detailing nature of illness of dependant,
ii. Knowledge and understanding of dependant’s medical history, iii. Personal contact between the employee and their supervisor/manager,
Proof Required. (a) For all absences the Employee must complete their leave form (and attach a medical certificate or statutory declaration where required by the Company under clause 23.4(b)) as soon as possible upon their return to work and seek approval for their absence from their immediate supervisor.
(b) Where the Company is not satisfied with an Employee’s stated reasons for the absence, the Company may require the Employee to provide a medical certificate from a registered health practitioner or where not reasonably practicable, a statutory declaration for any period of personal/xxxxx’s leave.
(c) In the event the Company is concerned that an Employee is developing an apparent pattern of absences over a reasonable period of time, the Company:
(1) may require the Employee to provide a medical certificate from a registered health practitioner or where not reasonably practicable, a statutory declaration, on all future occasions where personal/carer’s leave is claimed; and
(2) will review the amount of personal/xxxxx’s leave claimed by the Employee on an ongoing basis.
(d) Employees will not misuse their entitlement to personal/ carer’s leave.
Proof Required. Proof of an employee’s need for Emergency Carer’s Leave may be established through a number of means, including but not limited to: i Provision of medical certificates from a certified medical practitioner detailing nature of illness of dependant.

Proof Required. If the Housing Authority has any reason to suspect that a resident is housing an unauthorized guest or guests, the resident must prove to the Housing Authority that individual(s) has permanent residence elsewhere. If a resident doesn’t provide acceptable proof within 14 days from the date of the Housing Authority’s request, the Landlord may begin eviction proceedings for material noncompliance under Section 16 of the lease. Acceptable proof consists of two or more of the following: ✓ Valid lease with a valid receipt (for the current month); ✓ Copy of utility xxxx for the current month showing the person’s name and address (electric, gas, phone, cable); ✓ Current paycheck stub showing name and address; ✓ Current bank statement (savings and/or checking) showing name and address; ✓ Copy of mortgage coupon showing name and address; and ✓ Deed to a house, if owned, showing name and address. The following proof will not be accepted: ✓ Driver’s license ✓ Phone book listing ✓ Tax forms; and ✓ Other items which in the Authority’s view are not likely to provide a current address.
