Queue discipline. The purpose of an appointment system is to see patients in the order of their appointment times. So, if patients are punctual, they will simply be seen in the order of their appointment times. However, patients are not always punctual. To avoid keeping doctors idle while waiting for a patient to arrive, the actual order of seeing patients can differ from the order of their appointment times. When other waiting patients are present, it is possible that they will be seen first. That way, the risk exists that patients will start ignoring appointments and will start coming earlier than necessary in the hope of being served first.
