Refueling Services. If you would like us to put fuel in your boat as part of your launch, we are happy to oblige. Please note that to do this, you will need to complete a Credit Card Authorization Form. Fueling time is billed at our normal labor rate of $125/hour and your card will be charged for the fuel and fueling time at the time of service.

Refueling Services. You may choose one of the following refueling options: a) (Prepaid Fuel) The prepaid fuel option means prepaying for a full tank of fuel at the rental location. The price is based on fuel tank capacity of the vehicle rented multiplied by the per gallon charge of the renting location. Additional applicable taxes and surcharges may apply to the prepaid fuel option. This charge may change at the time of rental due to current fuel prices. No refund or credit will be given for unused fuel. If you do not want the prepaid fuel option, you can choose to return the tank full or pay for fuel used and not replaced when you return to the facility. This option must be chosen at the beginning of Rental and is not available at all locations. b) (Return Full or Pay) If you do not elect the Fuel Service Option, you may return the Vehicle with the same amount of fuel as when you received it. If you do not return the vehicle with the same amount of fuel as when you received it as noted on the fuel gauge at the time of rental, the Fuel Charge will be the per gallon rate not to exceed 150% of the local market price of self-service gasoline multiplied by National' s estimate of the number of gallons required to return the tank to the level as when received. This estimate is arrived at by using the manufacturer specified tank capacity for the Vehicle and then determining the capacity of the portion of the tank to return the tank to the level of when received. In any event, the Fuel Charge shall not exceed the price permitted under state law.
Refueling Services. Lessee, agrees to provide aviation fuels (100LL and Jet A) at the existing fuels tanks and fuel pumps at the airport. Lessee, agrees at its sole cost to replace the existing County owned single wall 100LL fuel tank with a suitable sized, double wall tank and shall maintain a card lock system for both types of fuels that allows 24/7 self-service, credit-card fueling. (a) Lessee also agrees at its sole cost and expense will maintain fuel tanks for each type of fuel to include but not limited to, tanks, fittings piping, pumps, fuel hoses, filters, and fuel dispenser. Xxxxxx further agrees periodic compliancy audits, of tanks, equipment inspections, and fuel testing is performed as outlined in ATA Specification 103, Standard for Jet Fuel Quality Control at Airports and Title 14 Code of Federal Regulations (CFR) part 139. (b) Lessee, shall operate and maintain the fuel storage and fuel supply facility at the airport. Lessee shall assume full responsibility for compliance with all federal, state, and local laws, rules, and regulations which apply to the fuel facility. Lessee shall maintain in effect and, if required, post in prominent location, all necessary and /or required license and/or permits. Lessee shall ensure that all personnel engage in the operation of fueling systems are properly trained and that documentation of such training is complete and available to County for inspection. Training shall include fuel system emergency response procedures. Notwithstanding the foregoing, training requirements shall not apply to aircraft operators engaged on self-service. Lessee shall provide and maintain an approved Spill Prevention Plan for operation. Lessee will assume full responsibility for compliance with all federal, state and local laws, rules and regulations which apply to the Spill Prevention Plan. Lessor retains the right to periodically review the price being charged for fuel by Xxxxxx.

  • Counseling Services SUD therapy/counseling is a relationship between people that works in part because of clearly defined rights and responsibilities held by each person. As a client in the SUD counseling process, you have certain rights and responsibilities that are important for you to understand. There are also legal limitations to those rights that you should be aware of. I, as your SUD counselor, have corresponding responsibilities to you. These rights and responsibilities are described in the following sections. SUD therapy/counseling has both benefits and risks. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness and helplessness, because the process of therapy often requires discussing the unpleasant aspects of your life. However, therapy has been shown to have benefits for individuals who undertake it. Therapy often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. But, there are no guarantees about what will happen. Therapy requires a very active effort on your part. In order to be most successful, you will want to put into practice things we discuss outside of sessions. The first 2-3 sessions will involve a comprehensive evaluation of your needs. By the end of the evaluation, I will be able to offer you some initial impressions of what our work might include. At that point, we will discuss your treatment goals and create an initial treatment plan. You should evaluate this information and make your own assessment about whether you feel comfortable working with me. If you have questions about my procedures, we should discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another SUD professional for a second opinion. PROFESSIONAL FEES The standard fee for a 50-minute individual session is $125.00 and the standard fee for a 90 minute couples session is $175.00 You are responsible for paying the full amount at the time of your session, unless prior arrangements have been made. Payment must be made by cash, or credit card. If you refuse to pay your debt, I reserve the right to use an attorney or collection agency to secure payment. APPOINTMENTS Appointments will ordinarily be 45-60 minutes in duration, once per week at a time we agree on, although some sessions may be more, or less frequent as needed. The time scheduled for your appointment is assigned to you and you alone. If you need to cancel or reschedule a session, I ask that you provide me with 24-hours notice. If you miss a session without canceling, or cancel with less than 24-hour notice, my policy is to collect the full session fee of $125.00/individual or $175.00/couples. It is important to note that insurance companies do not provide reimbursement for cancelled sessions; thus, you will be responsible for 100% of the full rate. If it is possible, I will try to find another time to reschedule the appointment. In addition, you are responsible for coming to your session on time; if you are late, your appointment will still need to end at the scheduled time. Finally, I have the right to terminate treatment after 2 missed appointments or habitual tardiness. Thank you for understanding. INSURANCE I am not a participating provider for any insurance plan. If you would like, I will supply you with a receipt of payment for services, which you can submit to your insurance company for reimbursement. Please note that not all insurance companies reimburse for out-of-network providers. PROFESSIONAL RECORDS I am required to keep appropriate records of the SUD therapy/counseling services that I provide. Your records are maintained in a secured, encrypted, HIPAA compliant web-based system. I keep brief records noting that you were here, your reasons for seeking SUD therapy/counseling, the goals and progress we set for treatment, your diagnosis, topics we discussed, your medical, social, and treatment history, records I receive from other providers, copies of records I send to others, and your billing records. Except in unusual circumstances that involve danger to yourself, you have the right to a copy of your file. Because these are professional records, they may be misinterpreted and / or upsetting to untrained readers. For this reason, I recommend that you initially review them with me, or have them forwarded to another mental health professional to discuss the contents. You also have the right to request that a copy of your file be made available to any other health care provider at your written request.

  • Dining Services The Contract is for a space in a University Housing & Dining Services (“UHDS”) facility and not for a particular room or type of housing. By signing this Contract, you agree to accept your residence assignment, and understand this assignment may change. Once you receive a key to your assigned residence or move personal belongings into your room (whichever comes first) you are considered to have taken occupancy (“Occupancy”) and will incur charges. You also agree to familiarize yourself and comply with all University policies governing occupancy, including those set forth in this Contract and in the UHDS publication Student Policy and Information Guide, to be considerate of other residents and to respect the rights of others at all times. The Student Policy and Information Guide may be found at: xxxx://xxxx.xxxx/policy-guide, as well as in paper form when requested at the University Housing & Dining Services Administrative Offices.

  • WARRANTY – SERVICES The Contractor warrants and represents that all services to be provided the City under the Contract will be fully and timely performed in a good and workmanlike manner in accordance with generally accepted industry standards and practices, the terms, conditions, and covenants of the Contract, and all applicable Federal, State and local laws, rules or regulations. A. The Contractor may not limit, exclude or disclaim the foregoing warranty or any warranty implied by law, and any attempt to do so shall be without force or effect. B. Unless otherwise specified in the Contract, the warranty period shall be at least one year from the Acceptance Date. If during the warranty period, one or more of the above warranties are breached, the Contractor shall promptly upon receipt of demand perform the services again in accordance with above standard at no additional cost to the City. All costs incidental to such additional performance shall be borne by the Contractor. The City shall endeavor to give the Contractor written notice of the breach of warranty within thirty (30) calendar days of discovery of the breach warranty, but failure to give timely notice shall not impair the City’s rights under this section. C. If the Contractor is unable or unwilling to perform its services in accordance with the above standard as required by the City, then in addition to any other available remedy, the City may reduce the amount of services it may be required to purchase under the Contract from the Contractor, and purchase conforming services from other sources. In such event, the Contractor shall pay to the City upon demand the increased cost, if any, incurred by the City to procure such services from another source.

  • Transportation Services i) In the event that transportation services for a student served by CONTRACTOR pursuant to an Individual Services Agreement are to be provided by a party other than CONTRACTOR or the LEA or its transportation providers, such services shall be reflected in a separate agreement signed by the parties hereto, and provided to the LEA and SELPA Director by the CONTRACTOR. Except as provided below, CONTRACTOR shall compensate the transportation provider directly for such services, and shall charge the LEA for such services at the actual and reasonable rates billed by the transportation provider, plus a ten percent (.
