CONTRACTOR agrees to provide COUNTY with reports that may be required by County, State or Federal agencies for compliance with this Agreement including and not limited to: • CONTRACTOR shall submit quarterly status reports and a final annual report to COUNTY which reflect progress made in implementing the services and achieving any outcomes set forth in the Scope of Services exhibit, and to assure CONTRACTOR’S compliance with contract terms. Said annual report shall be submitted by August 31 for the preceding fiscal year. • CONTRACTOR shall make annual client outcome information available to COUNTY within 60 days of fiscal year end. Outcome data will be based upon the full array of services provided and how those services advanced the functional improvement of the client. Functional improvement will be measured by the disposition of the client at discharge. COUNTY will inform CONTRACTOR of any changes related to funding sources or amounts in this agreement as a result of COUNTY’s Quarterly funding reviews. If changes are needed to reflect updated Federal Funding, this Exhibit is subject to modification with written approval of the County Contract Administrator and the Revenue and Budget Manager, and CONTRACTOR will receive the updated Exhibit.

XXX000000 BEGINS: April 1, 2021 ENDS: June 30, 2022 ADMINISTERING AGENCY: Health and Human Services, Public Health This is an Agreement made and operative as of the 1st day of April 2021, between the COUNTY OF PLACER, through its Health and Human Services Department, a political subdivision of the State of California, hereinafter referred to as “COUNTY”, and Lincoln Police Department, a government agency, hereinafter referred to as “CONTRACTOR.”

Key performance measures will include: • Activities conducted including the number of individuals reached and/or served. • Number of submitted and complete applications by program and method of application (paper or online). • Enter demographic data on monthly log • Line item detail documentation must be submitted on the Education Enrollment, Assistance and Retention Log (Monthly Activity Form) and attached to the fiscal activity report.

Reporting Exhibit. CONTRACTOR agrees to provide COUNTY with Full Psychological Reports, including recommendations, to deliver to the Courts when ordered by the Court.
Reporting Exhibit. Client Records: Original documentation shall not be removed from a hospital site by CONTRACTOR staff. The original documentation shall be placed in the locked bags provided at the site to ensure secure transportation of documents to the CONTRACTOR’S clinical site. The CONTRACTOR shall ensure that copies of all Mental Health Crisis Evaluations are submitted, in a secure manner, to the designated System of Care (Adult System or Care or Children’s System of Care) within one (1) business day. Medical Records: CONTRACTOR shall maintain and preserve all clinical records related to the resulting Agreement for ten (10) years from the date of discharge for adult clients, and records of clients under the age of eighteen (18) at the time of treatment must be retained until either one (1) year beyond the client’s eighteenth (18th) birthday or for a period of seven (7) years from the date of discharge, whichever is later. CONTRACTOR shall also contractually require the maintenance of such records in the possession of any third party performing work related to the resulting Agreement for the same period of time. Such records shall be retained beyond the seven-year period, if any audit involving such records is then pending, until the audit findings are resolved. The obligation to ensure the maintenance of the records beyond the initial seven (7) year period shall arise only if COUNTY notifies CONTRACTOR of the commencement of an audit prior to the expiration of the seven (7) year period. Financial Records: Adequate financial records shall be kept by CONTRACTOR so that they clearly reflect the cost of each type of service for which payment is claimed. Apportionment of costs shall be made in accordance with acceptable accounting principles in order to reflect the true cost of the service rendered for which payment is claimed. Independent Fiscal Audit: Within six (6) months of close of each COUNTY fiscal year, CONTRACTOR shall conduct and submit a fiscal audit for the period of this Agreement, as performed by an independent auditor, selected and performed in accordance with Generally Accepted Accounting Principles.

CONTRACTOR shall complete an Annual Report as required by CDSS. XXXXXXXXXX agrees to meet quarterly with county ILP Program Manager to review progress of achieving shared goals. CONTRACTOR to submit quarterly reports to county by the 15th of the month following the quarter to include the following: number of youth seen at Sierra College campus; work with CAPC personnel as appropriate to meet youth needs, number of Tahoe-placed youth receiving ILP services, any activities of engagement with Student Support Services, and other parties. CONTRACTOR will submit an annual report summarizing pre- and post-outcome data from the XXXXXX- XXXXX measurements taken during that time period.
