Resident Requirements. Resident agrees to follow all of the University’s guidelines and requirements regarding the continued maintenance of the Premise by Facilities staff members, including, without limitation, performing additional sanitation and hygiene measures, maintaining social distancing requirements from staff members, and meeting face mask/covering requirements while staff are present within the Resident’s space. The Resident recognizes that due to the nature of communal living and Facilities work, not all maintenance can be scheduled in advance. The Resident will work with Facilities to reschedule any non-emergent work if the Resident is otherwise occupied during the initial endeavor.

Resident Requirements. To meet the goal of long-term sobriety and self-sufficiency, requirements for being a Resident include and are not limited to: 1) Working with a sponsor/mentor one time per week 2) Attending 3 recovery meetings per week 3) Completing house chores on a daily basis when applicable 4) In first 30 days of residency: participating for at least 24 hours a week of any combination of the following: a) Outpatient treatment b) Attend an accredited school c) Being gainfully employed d) Volunteering *Beyond the first 30 days of residency, 24 hours becomes 32 hours of any combination of the above. Weekly participation of the above will be tracked on a weekly accountability form and managed by HM.
Resident Requirements. Sponsoring Institution represents and warrants that it performs a criminal background investigation on each individual accepted into its residency program and reviews the results of such background check prior the start of the student’s rotation at Participating Institution. The criminal background check shall cover all states of legal residence within the past five (5) years as reported by the resident. Upon receipt of any background check information that reveals a conviction, Sponsoring Institution will go through the suitability analysis set forth in _______ Health Care System LLC’s XXXX policy to determine appropriateness of placement and assign only those residents whom the Sponsoring Institution deems appropriate for the rotation based on the criteria set forth in such Policy. A copy of the applicable XXXX Policy is attached hereto as Exhibit B and incorporated herein by reference. In addition, Sponsoring Institution represents and warrants that each resident assigned to the Participating Institution is currently licensed to engage in the practice of medicine or podiatry under the laws of the (Name of State) and that said license and registration have not been suspended, revoked or restricted in any manner for any reason. Sponsoring Institution shall provide Participating Institution with assurance that the results of the resident’s criminal background check are suitable as well as evidence of (Name of State) licensure upon request.
Resident Requirements. Residents hereby represents, warrants, and acknowledges to and covenants that Resident shall: a. Minimum/Maximum Temperature. Maintain the air conditioning system (“A/C”) in your Apartment at or below 77 degrees Fahrenheit at all times. Resident shall remain solely liable for any electrical or utility expense related to Owner’s temperature requirements; b. If applicable, maintain the humidistat setting at 80 degrees at all times. c. Never run the fan in the manual “on” position for a period of more than thirty (30) minutes time. d. IMMEDIATELY report any A/C malfunction to the front office; e. Promptly inform the front office if Resident will be away from the Apartment for a period longer than three (3) days, so the A/C may be property monitored by the front office; f. Keep closet doors open at all times if Resident will be away from the Apartment for a period longer than three (3) days, so air can continuously circulate properly throughout the Apartment. g. Keep all vents in your Apartment open at all times; and, h. Keep any and all doors and vents free from any type of obstruction. i. Never store anything in front of ventilation doors to your A/C unit; j. Never block the ventilation doors to your A/C unit; k. Check at least monthly the cabinets beneath your sinks for any leaks.
