Schedule of Evaluation. 1. A minimum of two (2) formal observations shall be conducted to support each performance assessment/evaluation. A formal observation shall last a minimum of thirty (30) minutes. There shall be a minimum of three (3) weeks between formal observations. If after the second formal observation, a counselor’s performance is found deficient to the extent that adverse personnel action may result, a minimum of one (1) additional observation shall be conducted.
