SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The CITY has employed the CONSULTANT to provide <WRITE OUT THE SCOPE OF WORK CONSULTANT WILL PERFORM IN DETAIL, INCLUDING THE TIME FRAME AND MILESTONES; THE SCOPE CAN ALSO BE ATTACHED AS AN EXHIBIT.> The services to be provided include the scope of work in RFP # FY 2015‐ 2016‐004 COST RECOVERY SERVICES FOR TRAFFIC IMPACT ANALYSIS CCNA, which is hereby incorporated and made part of the is agreement by reference and the Proposal submitted by CONSULTANT, which is hereby incorporated and made part of this Agreement by reference.
SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The CITY has employed the CONTRACTOR to provide the work which includes the scope of work in RFP # FY 0000-0000-000 Disaster Debris Monitoring Services and Proposal submitted by CONTRACTOR, which is hereby incorporated and made part of this Agreement by reference.
SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The CITY has employed the CONTRACTOR to provide processing for materials delivered by the City to their site. The City will deliver materials to the following site: CONTRACTOR located at 0000 XX 00xx Xxxxxxx, Xxxxx Xxxxx, XX 00000. Materials will be delivered by City Equipment or City contractor, by mixed load or clean load for yard waste: Vegetative Materials, Clean Wood, Storm Debris (Vegetative and Horticultural and Stumps Exceeding 24”) Bulk Materials, Construction & Demolition Debris, Source Separated Materials (ISS), and Street and Earth Debris. For the purpose of this contract, the definitions for materials contained in this section shall apply unless otherwise specifically stated. Bulk items include, but not limited to, appliances, furniture, console televisions and other similar items.
SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The work to be provided includes the scope of work in RFP # FY 0000-0000-000 GROUP LIFE INSURANCE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D), SUPPLEMENTAL LIFE INSURANCE AND LONG-TERM DISABILITY INSURANCE FOR CITY OF HALLANDALE BEACH. Title of Project and Proposal submitted by CONTRACTOR, which is hereby incorporated and made part of this Agreement by reference.
SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The CITY has employed the CONTRACTOR to provide the scope of work in Bid # FY 0000-0000-000 – Police Department Uniforms Purchase and Proposal submitted by CONTRACTOR, which is hereby incorporated and made part of this Agreement by reference. Community Benefit Plan – only applicable if the project cost is over $1 million. CONTRACTOR shall comply with the Community Benefit Plan (CBP) attached hereto as Exhibit “B”. A form shall be developed by the CONTRACTOR and provided to the Project Manager, called Exhibit “D” which must be utilized to track the work force utilized for the CBP. Exhibit D shall be submitted to the Project Manager monthly at a day and time as agreed. Weekly reporting and monthly meetings will be held between the CITY and the CONTRACTOR to review and monitor the compliance of the CBP. Non-compliance of the CBP will result in up to $25,000 in payment due to be held by CITY until compliance is reached.

The work to be provided includes the scope of work in RFP # FY 0000-0000-000 CITY OF HALLANDALE BEACH GROUP MEDICAL WITH PHARMACY, MEDICAL GAP PLAN, DENTAL, VISION, EMPLOYEE ASSISTANCE PROGRAM, FLEXIBLE SPENDING ACCOUNT AND COBRA ADMINISTRATION. Title of Project and Proposal submitted by CONTRACTOR, which is hereby incorporated and made part of this Agreement by reference.
SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The CITY has employed the CONTRACTOR to provide the work which includes the scope of work in Bid # FY 0000-0000-000 POST DIASTER CATERING SERVICES and Proposal submitted by CONTRACTOR, which is hereby incorporated and made part of this Agreement by reference.
SCOPE OF SERVICES TO BE PROVIDED TO THE CITY. The CITY has employed the CONTRACTOR to provide services stated in the scope of work in RFP # FY 0000-0000-000 GISND UNARMED SECURITY GUARD SERVICES AND ROVER PATROL and Proposal submitted by CONTRACTOR, which is hereby incorporated and made part of this Agreement by reference as Exhibit A. The Contractor will charge the following cost for the transponders and any increases to the cost of the transponders for the duration of the Agreement. Any changes to the price of the transponders must be approved by the GISND Liaison, Designee and/or City Contract Administrator.
