Strengthen Incident Management. (Capability 3) 1. New PHRCs, Public Health Nurse Managers (PHNM), and Health Department Directors (HDD) will attend CRT and fiscal training within thirty (30) days of hire. 2. Submit the quarterly match summary by completing the match spreadsheet on the CRT. Submit supporting documentation from the PHNI system if PHPR time is tracked or another method if the Subrecipient does not use PHNI to track PHPR support and upload to CRT. Due dates are Q1 due October 31, 2024; Q2 due January 31, 2025; Q3 due April 30, 2025; and Q4 due July 31, 2025.
Strengthen Incident Management. 1. The Provider shall develop and establish a standardized incident command structure that is consistent with the National Incident Management System (NIMS) and use the National Response Framework (NRF) to guide emergency operation planning. The Provider shall use incident command structure to implement elements of the NRF in scalable and flexible ways. The Provider shall provide evidence that employees designated as incident command and support personnel demonstrate progress toward attainment of minimum NIMS course completion as defined by the Federal Emergency Management Agency and the Department. The Provider shall develop and implement a course completion tracking mechanism that is able to be submitted to the Department when requested and made available for review during site visits. This task shall be completed throughout the term of the Agreement.

  • Incident Management 3.1. We shall notify You without undue delay after We becomes aware of any accidental or unlawful destruction, loss, alteration, unauthorized disclosure of, or access to Customer Data, including Personal Data, stored or otherwise processed by Us or Our sub-processors of which We become aware (“Security Incident”).
