STUDENT AGREES TO. Register for the appropriate course if approved for credit. Work an average of Click or tap here to enter text.hours/week during the internship semester. Develop a well-planned series of learning objectives, in conjunction with the faculty coordinator or intern staff and the employer. The objectives should be related to the goals of the student's instructional programs. Immediately notify the Career and Professional Development office and faculty coordinator of any problems or changes in job responsibilities. Abide by the regulations and policies of both the Experiential Education Program & the employer. Remain employed the entire semester or risk the loss of credits. Drop all credits if he/she leaves the Experiential Education program without the consent of the Career and Professional Development office or if he/she is discharged from the job prior to the completion of the required amount of time in the job.

Not release any information that may be confidential without verification that the release is authorized. C. Follow any and all licensed health care facility procedures for dealing with confidential information, including the destruction of such information. D. Keep computer access password a secret, and to not use anyone else’s computer access password, if applicable. E. Notify supervisor of any known or suspected misuse of confidential information.
STUDENT AGREES TO a) Be available and ready for each scheduled lesson 10 minutes prior to, and up to 30 minutes after the scheduled lesson time. b) Give School at least 48 hours notice of any schedule change. c) Practice driving skills as suggested by Instructor as much as possible between lessons. d) Notify School of any and all health issues including any relevant illness or injury, vision or hearing issues, mental disorders (OCD, ADHD, etc.) or physical disabilities or impairments. e) Wait for, and follow Instructor’s instructions at all times. f) Carry their valid Learner’s Permit or Driver’s License at all times when driving on public roads.
