Submission of Claim Form. The NYC Class Member must submit a Claim Form to the Settlement Administrator within the Acceptance Period and no later than the Claim Bar Date, pursuant to Section II.G above, in order for the Claim Form to be valid and for the NYC Class Member to be eligible for consideration to receive an Enhanced Payment. The NYC Class Member may validly submit a Claim Form through e-mail, fax, U.S. mail (via the pre-paid envelope referenced in Section VIII.B) or the Settlement Website.

Claim Forms will be made available by mail and for downloading from the Settlement Website maintained by the Claims Administrator and may be made available on the websites of Plaintiffs' Lead Counsel. Class Counsel, Defendant Coinbase, and Defendant Coinbase’s Counsel shall have access, upon request, to the information submitted by Class Members to the Claims Administrator but will keep confidential the information provided by Class Members on Claim Forms and will not use the information for any purpose other than in connection with Claims Administration and will not share, distribute, or disclose such information.

A Claim Form must be submitted by regular First Class U.S. Mail, fax, or by electronic mail in Portable Document Format ("PDF") to the Claims Administrator, or such other method approved by the Court. Claim Forms will be made available by mail and for downloading from the Settlement Website maintained by the Claims Administrator and may be made available on the websites of Plaintiffs' Lead Counsel. Ridgewood’ Counsel agrees to keep confidential the information provided by Class Members on Claim Forms and to not share, distribute, or otherwise make available to Xxxxxx or any person or entity acting on their behalf of otherwise representing them such materials. Xxxxxxxxx shall not be allowed to view or receive copies of any information provided by Class Members.
Submission of Claim Form. All Claim Forms shall be signed under penalty of perjury and sent directly to the Settlement Administrator at the address indicated on the Claim Form. All claim forms seeking refunds shall be accompanied by a return of the Covered Products, original retail receipts, and/or a Claim Form signed under penalty of perjury. The Settlement Administrator shall review the Claim Forms and make any calculations of payments to be distributed to the Settlement Class Member.
Submission of Claim Form. In order to receive a payment under this Agreement, Settlement Class Members must not submit an Opt-Out Request and must submit a complete and executed Claim Form to Class Counsel. The Claim Form must actually be received by Class Counsel before the expiration of the Notice Period. Class Counsel (will notify any Settlement Class Member if it receives a Claim Form from them that is not timely and/or valid.
