Submission of Claim Form. 5.1 In order to be eligible to receive any share of the Global Settlement Amount, a Settlement Class Member must submit a valid Claim Form and Release to the Claims Administrator no later than the later of (i) sixty (60) days from the date Mail Notice is completed,
Submission of Claim Form. The NYC Class Member must submit a Claim Form to the Settlement Administrator within the Acceptance Period and no later than the Claim Bar Date, pursuant to Section II.G above, in order for the Claim Form to be valid and for the NYC Class Member to be eligible for consideration to receive an Enhanced Payment. The NYC Class Member may validly submit a Claim Form through e-mail, fax, U.S. mail (via the pre-paid envelope referenced in Section VIII.B) or the Settlement Website.
Submission of Claim Form. Each Settlement Class Member shall sign and submit a completed Claim Form in the form of Exhibit A hereto together with all other documentation, if any, set forth below either online or on the Settlement Website. A Claim Form must be submitted by regular First Class U.S. Mail, fax, or by electronic mail in Portable Document Format ("PDF") to the Claims Administrator, or such other method approved by the Court. Claim Forms will be made available by mail and for downloading from the Settlement Website maintained by the Claims Administrator and may be made available on the websites of Plaintiffs' Lead Counsel. Ridgewood’ Counsel agrees to keep confidential the information provided by Class Members on Claim Forms and to not share, distribute, or otherwise make available to Xxxxxx or any person or entity acting on their behalf of otherwise representing them such materials. Xxxxxxxxx shall not be allowed to view or receive copies of any information provided by Class Members.
Submission of Claim Form. Any Settlement Class Member who did not submit a valid and approved claim in the Cryptsy Case shall follow the claim submission procedures set forth in this paragraph. Except as provided above in Section IV(B)(2) above, to receive a distribution from the Settlement Fund, a Settlement Class Member must sign and submit a completed Claim Form in the form of Exhibit A hereto together with all other documentation, if any, set forth below either online or on the Settlement Website. Submission of Claim Form. The Settlement Class Member shall sign and submit to the Claims Administrator a completed Claim Form, in the form of Exhibit 8 hereto, together with all other documentation, if any, set forth below, by regular First Class mail, fax, electronic mail, or via the settlement website.
Submission of Claim Form. All Claim Forms shall be signed under penalty of perjury and sent directly to the Settlement Administrator at the address indicated on the Claim Form. All claim forms seeking refunds shall be accompanied by a return of the Covered Products, original retail receipts, and/or a Claim Form signed under penalty of perjury. The Settlement Administrator shall review the Claim Forms and make any calculations of payments to be distributed to the Settlement Class Member.
Submission of Claim Form. In order to receive a payment under this Agreement, Settlement Class Members must not submit an Opt-Out Request and must submit a complete and executed Claim Form to Class Counsel. The Claim Form must actually be received by Class Counsel before the expiration of the Notice Period. Class Counsel (will notify any Settlement Class Member if it receives a Claim Form from them that is not timely and/or valid.
Submission of Claim Form. Class Members who wish to make a claim must submit a Claim Form to the Claims Administrator, either online or by mail, within 12 months after the Mailing Deadline. A proposed Claim Form is attached as Exhibit A.
Submission of Claim Form. Members of the Claims-Made Class who wish to receive the Award must submit to the Settlement Administrator a completed Claim Form on or before the close of the Claims Period. The Claim Form will be mailed to the Class Members and also made available on the Settlement Website for Class Members to print and deliver to the Settlement Administrator by mail or e-mail. The Settlement Website will also contain a Claim Form that can be completed online, electronically signed, and submitted to the Settlement Administrator online.
