Supplemental Health and Welfare Benefits. A. Life: Coverage of $50,000 plus $50,000 accidental death: Town to pay for premium. Additional life insurance may be purchased by the employee equal to once or twice the employee's base salary, not to exceed $200,000.
Supplemental Health and Welfare Benefits. 13.6.1 Life - Coverage of $50,000 plus $50,000 accidental death: Town to pay for premium. Additional life insurance may be purchased by the employee equal to once or twice the employee's base salary, not to exceed $200,000. 13.6.2 Disability - Short Term Disability Policy effective 8th calendar day; coverage of 60% of weekly earnings up to maximum of $1,300/week for 13 weeks. Long Term Disability benefits begin on the 91st day of disability; coverage of 60% of monthly earnings up to a maximum of $6,000/month. 13.6.3 Employee Assistance Program - Premiums shall be paid by the Town. 13.6.4 Unemployment Insurance - Premiums shall be paid by the Town to provide State coverage.
