Type Selection Report. A-E may be required to prepare engineering studies, traffic safety investigations, and reports, in conformance with industry standards and County of Orange and State Report Format. Deliverables shall include the A-E’s reviewing of existing documents, visiting project site, identifying and evaluation conceptual design issues based on existing and ultimate conditions, preparing preliminary calculations and cost estimates, identifying access and detour concepts and determining all major issues affecting budget, construction phasing, prioritization and schedule. Also, the report shall recommend the most practical, feasible and cost-effective solution and justify the chosen alternative. To ensure proper project development, A-E shall attend and conduct project status meetings with staff and with others as required to discuss status and/or details of project.

Type Selection Report. Prepare a Type Selection Report that summarizes our recommended bridge type, which is best suited to the preferred project alignment as determined in the Project Report. Items that will be addressed in this report include other viable bridge types, abutment and bent layout, utility issues, maintenance issues, aesthetic issues and construction methods. This report will reference the preliminary construction cost estimates for other bridge types and bridge configurations studied for this project. The Type Selection Report will be made available to the City and Caltrans. We will attend an informal Type Selection Meeting at the City of Chula Vista. If desired, the City can invite a representative from Caltrans Local Assistance. However, since our project is not within Caltrans Right of Way, our scope does not include a formal Type Selection Meeting with Caltrans in Sacramento. The draft and final Type Selection Report will be provided to the City.

Type Selection Report. CONSULTANT will prepare a Type Selection Report to summarize the results of the concept development and alternatives analysis, and make a recommendation in regard to the selected structure type for the bridge replacement as well as a recommendation for the proposed retaining walls. The report will include discussion of the restrictions of the existing narrow roadway and materials transportation to the site, preliminary hydraulic, and geotechnical studies, and will consider environmental, right-of-way, constructability, and construction cost impacts. The format and content of the type selection memo shall be as described in Caltrans Bridge Memos to Designers Manual. The General Plans and General Plan Estimates for the 2 bridge design alternatives and retaining wall alternatives will be included in the Type Selection Report. The chosen alternative, once reviewed and approved by the COUNTY and Caltrans Local Assistance, shall be the basis for the final design of the structure. Task 6.3.1: Revise Type Selection Report, Revision 2 Given the changes to the site, CONSULTANT will revise the Type Selection Report to reflect the changes to the bridge alignment, bridge width, barrier rail, and aesthetic features such as the sacrificial arch member.

  • Implementation Report Within 150 days after the Effective Date, Halifax shall submit a written report to OIG summarizing the status of its implementation of the requirements of this CIA (Implementation Report). The Implementation Report shall, at a minimum, include: 1. the name, address, phone number, and position description of the Compliance Officer required by Section III.A.1, and a summary of other noncompliance job responsibilities the Compliance Officer may have; 2. the names and positions of the members of the Compliance Committee required by Section III.A.2; 3. the names of the Board members who are responsible for satisfying the Board of Commissioners compliance obligations described in Section III.A.3; 4. a copy of Halifax’s Code of Conduct required by Section III.B.1; 5. the number of individuals required to complete the Code of Conduct certification required by Section III.B.1, the percentage of individuals who have completed such certification, and an explanation of any exceptions (the documentation supporting this information shall be made available to OIG upon request); 6. a summary of all Policies and Procedures required by Section III.B (copies of the Policies and Procedures shall be made available to OIG upon request); 7. the following information regarding each type of training required by Section III.C: a. a description of such training, including a summary of the topics covered, the length of sessions, and a schedule of training sessions; b. the number of individuals required to be trained, percentage of individuals actually trained, and an explanation of any exceptions; and c. with respect to active medical staff members, the number and percentage who completed the training, the type of training and the date received, and a description of Halifax’s efforts to encourage medical staff members to complete the training. 8. a description of: (a) the Focus Arrangements Tracking System required by Section III.D.1.a; (b) the internal review and approval process required by Section III.D.1.e; and (c) the tracking and monitoring procedures and other Focus Arrangements Procedures required by Section III.D.1; 9. a description of the Disclosure Program required by Section III.F;
