VLTP Leave Recipient a. An employee may complete and submit, for his/her front-line manager’s approval, an electronic or manual VLTP leave recipient application for an approved period of leave without pay for a personal medical emergency or to care for a family member with a personal medical emergency. If an employee is not capable of making an application, a personal representative of the potential leave recipient may complete and submit an application on the employee's behalf. Each application shall be accompanied by a written (or electronically prepared) manager-approved leave request and medical certification. The information required in the medical certification is described in HRPM LWS-8.1, Sick Leave for Personal Medical Needs, established June 28, 2004 and effective June 13, 2011,and HRPM LWS-8.2, Leave Options to Care for a Family Member, established June 28, 2004 and effective April 25, 2010.

  • Maternity/Child Care Leave A. The Board agrees to provide employees with a child care leave of absence, without pay, as set forth below:

  • Uniformed Service Shared Leave Pool Eligible state employees may donate leave to the uniformed services shared leave pool for use by state employees who have been called to active duty in one of the uniformed services of the United States.   • Cyclic Employment Leave Leave without pay will be granted to cyclic year employees during their off season.

  • Reservist Leave 23.32 An employee who is a military reservist is entitled to take a leave of absence without pay if they are deployed to a Canadian Forces operation outside Canada, or to a domestic operation that is or will be providing assistance in dealing with an emergency or its aftermath (e.g. a search and rescue operation or a natural disaster response).

  • Union Leave of Absence An employee on an unpaid Union leave of absence shall have her wages, benefits and seniority continued by the Employer, and the Union agrees to reimburse the Employer for the costs of such wages and benefits. Employees requesting leave under this article will provide the Employer with as much advance notice as possible of the dates of the leave. Where there are less than fifteen (15) regular employees at a workstation at the time the leave request is submitted, and subject to operational requirements, unpaid Union leave of absence will be granted to one employee for the purpose of conducting Union business. This would be an additional person on Union leave at worksites where the position of the Union President or Council member has been backfilled for the duration of their term of office. A leave of absence without pay shall be granted to an employee who is a member of the Union and who is:

  • Medical Leave of Absence Where you have a medical leave of absence due to any medically determinable physical or mental impairment that can be expected to result in death or can be expected to last for a continuous period of not less than six months, and you have not returned to employment with the Company or an Affiliate, a Separation from Service has occurred on the earlier of: (A) the first day on which you would not be considered “disabled” under any disability policy of the Company or Affiliate under which you are then receiving a benefit; or (B) the first day on which your medical leave of absence period exceeds 29 months.

  • Bereavement Leave of Absence An employee who has a bereavement in their immediate family will be granted up to and including five (5) days absence with pay. Immediate family means spouse, (as defined in Section 2(b) of Appendix 2), parents, and child. For this provision “step” relationships are considered as immediate family. An employee who has a bereavement in their family will be granted up to and including three (3) days absence with pay. Family means brother, sister, parents-in-law, grandparents, grandchildren, son-in-law and daughter-in-law. For this provision “step” relationships are considered as family. In the event of the death of an employee's brother-in-law, sister-in-law or grand parents- in-law the employee will be given one (1) days' leave with pay to attend the funeral. An additional day's traveling time, in each direction, will be granted in order for the employee to attend the funeral if such is to be held in excess of 600 kilometers from the employees’ home.

  • Employment During Unpaid Maternity Leave (a) Special Temporary Employment

  • Child Care Leave (a) An employee who is a natural or adoptive parent shall be granted upon request in writing child care leave without pay for a period of up to thirty-five (35) weeks. The leave may be shared by the parents or taken wholly by one (1) parent.
