Xxxx Management. A collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services (both Medicare and Medicaid) required to meet an Enrollee’s needs across the continuum of care. It is characterized by advocacy, communication, and resource management to promote quality, cost effectiveness, and positive outcomes.

Xxxx Management. Vendor will perform the following XXXX management functions: a. In addition to working with the Software supplier (“Publisher”) on pricing and other commercial requirements, Vendor will provide to Publisher the Governmental Entities custom XXXX terms (“Customer XXXX Terms”) or the Publisher’s XXXX terms, as modified by the Governmental Entities. b. If the Publisher agrees with and countersigns the Custom XXXX Terms or the XXXX terms as modified by Governmental Entities, Vendor will notify Governmental Entity to proceed with purchase of software titles by placing the Purchasing Instrument. c. If the Publisher rejects the Custom XXXX Terms or the modified XXXX terms, Vendor will notify Governmental Entity and Governmental Entity will elect to either continue with the Purchasing Instrument, decline to proceed, or engage in negotiations directly with the Publisher, all as facilitated/coordinated by Vendor. d. If the Governmental Entity accepts the original Publisher XXXX terms, the Governmental Entity will notify Vendor in writing or proceed with placing the Purchasing Instrument. e. If the Governmental Entity rejects the original Publisher XXXX terms, Vendor will provide a new Publisher and corresponding XXXX.
Xxxx Management. Prompt resolution of any dispute is important to both Parties and the Parties agree that the arbitration of any dispute shall be conducted expeditiously. The arbitrator is instructed and directed to assume case management initiative and control over the arbitration process (including scheduling of events, pre-hearing discovery and activities, and the conduct of the hearing), in order to complete the arbitration as expeditiously as is reasonably practical to obtain a just resolution of the dispute.
Xxxx Management. The Contractor shall provide Care Management activities to appropriate Enrollees as described in this Section and further specified by EOHHS. 1. The Contractor shall proactively identify Enrollees who may benefit from Care Management activities based on the results of a systematic evaluation as described in this Section. Such evaluation shall: a. Explicitly incorporate, at a minimum: 1) Enrollees with Special Health Care Needs; 2) Enrollees with LTSS needs as indicated by the results of the Care Needs Screening described in Section 2.3.B.2.g; 3) Enrollees who are identified by EOHHS as potentially in need of Care Management; 4) Enrollees who are identified by PCCs as potentially in need of Care Management; and 5) Enrollees who self-identify to the Contractor as potentially in need of Care Management; b. Incorporate information contained, if applicable and as available, in each Enrollee’s: 1) Care Needs Screening; 2) Claims or encounter data; 3) Medical records; 4) Laboratory results; 5) Pharmacy data; 6) Discharge data; and 7) Other relevant sources of information identified by the Contractor or EOHHS; and c. Incorporate predictive modeling of an Enrollee’s risk for high cost, high utilization, admission, re-admission, or other adverse health outcomes. 2. The Contractor shall provide each identified Enrollee with Care Management as follows: x. Xxxx Management shall include, but not be limited to, activities such as: 1) Providing a Comprehensive Assessment as described in Section 2.3.D.1 for Enrollees assigned to a BH or LTSS CP; 2) Otherwise comprehensively assessing Enrollee’s with Special Health Care needs as described in Section 2.3.D; 3) Creating a documented Care Plan as described in Section 2.3.
Xxxx Management. 5.1 We reserve the right at our sole discretion, to (1) monitor the Site for breaches of these Terms and Conditions; (2) take appropriate legal action against anyone in breach of applicable laws or these Terms and Conditions; (3) remove from the Site or otherwise disable all files and content that are excessive in size or are in any way a burden to our systems; and (4) otherwise manage the Site in a manner designed to protect our rights and property and to facilitate the proper functioning of the Site and Services. 5.2 We do not guarantee that the Site will be secure or free from bugs or viruses. 5.3 You are responsible for configuring your information technology, computer programs and platform to access the Site and you should use your own virus protection software.
Xxxx Management. Maintain in effect at all times the cash management systems described in place on the Second Amendment and Restatement Effective Date or alternative cash management systems reasonably acceptable to the Administrative Agent.
Xxxx Management. The agent bank may inspect and supervise the borrower’s use of each loan fund at any time, and the borrower shall cooperate with the agent bank in loan payment management, post-loan management and related inspections. Provide for transfer of cash between accounts, make and accept wire transfers of funds in both USD and non-U.S. currency (including provision of Fed reference numbers upon request); 7. Provide for same-day settlement of cash trades, such as physical, DTC, and Fed wireable repo and term purchases. Cash flow from purchases and sales must be debited/credited on contractual settlement date; 8. Credit income earned, redemptions, and maturities to the proper accounts on payable date in Fed funds. The Contractor must identify any security types not eligible; 9. Provide a system that daily projects cash by account for a minimum of five days forward. Projections should incorporate STIF balances, dividend and interest income, and income from trade settlements, corporate actions, or other known cash flows; 10. Provide currently available cash balances to the Agency’s investment managers each morning by 10 AM Eastern Time; 11. Provide notification of overdrafts to the Agency and its investment managers daily; and 12. Complete and report foreign currency conversion and movement transactions
