If you feel unsure that you can make this promise, we ask that you do not use the facilities and services until we have completed your health and fitness profile.

b. You agree to give us all the relevant personal, health and fitness information both prior to the Membership and during the course of the Membership should your health status change. You also agree to complete our Pre-Exercise Questionnaire. In some cases, responses you give will require that you get medical advice and guidance before exercising. You acknowledge that pre-exercise or other screening is no substitute for medical advice and does not guarantee against injury or death.
c. You promise that information you provide the Club is true and accurate and is not misleading in any way.
d. You must not use our Facilities and Services if you suffering from any injury, illness, disease or other condition that could put your and other
e. We may suspend or cancel your Membership if we have reason to suspect that you have not complied with clause 5.1.
Your physical condition. 1. To ensure the Coach provides the most appropriate training regime for you, the facility requires you to complete a Pre-Exercise Questionnaire.
2. You warrant that as at the Training Start Date, you are in reasonable physical condition and that you have advised the facility and noted in the Schedule, any Medical Conditions, and any physical, mental or emotional health issues or other reasons (that you are aware of) why you cannot or should not take part in active or passive exercise.
3. You promise to notify True Technique about any changes in your physical, emotional and mental health, your htness and medical condition generally, so that your training regime can be appropriately modihed, and your condition monitored, to avoid or minimize risks of injury or detriment to your health generally.
4. Any advice and/or recommendation by the Coaches is not intended to constitute medical diagnosis or treatment, nor should it be relied on as a suggested course of medical treatment for a particular condition. If you are in doubt, you must obtain medical advice prior to commencing or continuing any exercise program and advise the Coach or facility if you feel any discomfort during or after a training session.
5. If you have an infection, contagious illness or physical ailment such as an open sore or cut, or if there is any other risk of infection or illness, however small, to any other person, you are not permitted to use studio facilities. You must not attend and use the facilities and services whilst you are suffering from any illness, disease, injury or other condition that could present a risk to the health or safety of other members and guests or yourself. If you happen to use the facilities and services before disclosing relevant health information to us, you promise that you are in good physical condition and you do not know of any reason why you may not be able to exercise safely. If you feel unsure that you can make this promise, we ask that you do not use the facilities and services until we have completed your health and fitness profile.
Your physical condition. By participating in the Experience you agree that:
(a) you do not know of any circumstances which would disqualify you from driving;
(b) you are not currently suffering from any physical or mental condition which would restrict or impair your alertness or driving ability and of which you have not informed us;
(c) your eyesight meets the standard required for driving. The course may be unsuitable and therefore not recommended for those who suffer from back problems, neck problems, heart conditions (or any other similar conditions). It may also be unsuitable for those who are pregnant.
