012 Background. Michigan’s public mental health system is currently providing supports and services for 32,000 individuals with I/DD. At this time, the needs of these individuals are determined by methods that are not consistent throughout the state. In order to ensure equity in service delivery and maintain the cost-effectiveness of the overall service system, the Behavioral Health and Developmental Disabilities Administration (BHDDA) has initiated a process to provide for assessment of the support needs of persons with I/DD through an empirically tested and validated tool which can be submitted to and is available in a secure web based system with an electronic format. The tool will be part of BHDDA efforts to support the person centered planning process and provide accurate assessment information that is consistently gathered and available throughout the state. Michigan will also use this assessment as part of an independent determination process to support the determination of need for the Medicaid benefit of comprehensive community living supports and services.

012 Background. Caro Center is a State of Michigan, Department of Community Health Psychiatric Hospital. Caro Center is located at 0000 Xxxxxxxx Xxxx, Caro, Michigan 48723. Caro Center has an average census of 160 patients with varying diagnosis and treatment plans. The Dentist provides varying degrees of care to the patients of the Caro Center. The Dentist is required to maintain a current license, maintain required patient documentation, follow hospital policies and procedures, and meet all State and Federal accrediting guidelines.
012 Background. The Applied Biosystems Genetic Analyzer and 7500 Fast Dx Real-Time PCR laboratory instruments are used to complete laboratory testing in the MDCH-BOL. The 7500 Fast Dx Real-Time PCR Instrument is used for pandemic influenza testing and for testing for the agents of bioterrorism for both human and environmental samples. The Genetic Analyzer is a sequencer provided to MDCH-BOL by the Center for Disease Control (CDC) for use in performing TB Genotyping under contract to the CDC. This analyzer also serves as surge capacity for pandemic influenza. Both instruments are highly sophisticated and require regular maintenance and service by qualified field service engineers.

  • Criminal Background Check It is the Responsibility of CHESAPEAKE CENTER, INC. to make certain that its employees, agents, volunteers, and contractors, who have contact with students receiving services are fingerprinted and have a background check in compliance with Family Law Article, Annotated Code of Maryland, and Section 5-551 through 5-557. CHESAPEAKE CENTER, INC. may not hire, contract, or otherwise engage an individual to participate in this Cooperative Agreement who has been convicted of a crime involving child abuse or neglect; contributing to the delinquency of a minor; a crime of violence as set forth in Criminal Law Article §14-101, Annotated Code of Maryland; or has evidence of a criminal history which in the opinion of Chesapeake Center, Inc. makes the individual unfit to participate in this Cooperative Agreement.

  • Background Check The Department or Customer may require the Contractor to conduct background checks of its employees, agents, representatives, and subcontractors as directed by the Department or Customer. The cost of the background checks will be borne by the Contractor. The Department or Customer may require the Contractor to exclude the Contractor’s employees, agents, representatives, or subcontractors based on the background check results. In addition, the Contractor must ensure that all persons have a responsibility to self-report to the Contractor within three (3) calendar days any arrest for any disqualifying offense. The Contractor must notify the Contract Manager within twenty-four (24) hours of all details concerning any reported arrest. Upon the request of the Department or Customer, the Contractor will re-screen any of its employees, agents, representatives, and subcontractors during the term of the Contract.

  • Background Checks The State may require that the Contractor and Contractor Parties undergo criminal background checks as provided for in the State of Connecticut Department of Emergency Services and Public Protection Administration and Operations Manual or such other State document as governs procedures for background checks. The Contractor and Contractor Parties shall cooperate fully as necessary or reasonably requested with the State and its agents in connection with such background checks.

  • Background Screening VENDOR shall comply with all requirements of Sections 1012.32 and 1012.465, Florida Statutes, and all of its personnel who (1) are to be permitted access to school grounds when students are present, (2) will have direct contact with students, or (3) have access or control of school funds, will successfully complete the background screening required by the referenced statutes and meet the standards established by the statutes. This background screening will be conducted by SBBC in advance of VENDOR or its personnel providing any services under the conditions described in the previous sentence. VENDOR shall bear the cost of acquiring the background screening required by Section 1012.32, Florida Statutes, and any fee imposed by the Florida Department of Law Enforcement to maintain the fingerprints provided with respect to VENDOR and its personnel. The parties agree that the failure of VENDOR to perform any of the duties described in this section shall constitute a material breach of this Agreement entitling SBBC to terminate immediately with no further responsibilities or duties to perform under this Agreement. VENDOR agrees to indemnify and hold harmless SBBC, its officers and employees from any liability in the form of physical or mental injury, death or property damage resulting from VENDOR’s failure to comply with the requirements of this section or with Sections 1012.32 and 1012.465, Florida Statutes.
