ACTIVITY CARDS. A.S.B. cards may be purchased in the cashier’s office or online at xxxxx://xx-xxxxxx.xxxxxxxxxxxxxxxxx.xxx/ The A.S.B. sticker may offer reduced prices for dances/events. The A.S.B. sticker is required for any student participating in co-curricular activities. A.S.B. cards are $20 for 6th graders and $35 for 7th and 8th graders.

ACTIVITY CARDS. The District agrees to furnish each employee with activity cards which will permit the employee and employees spouse and immediate family members who are enrolled in elementary, middle/junior high, or high school to attend home games free of charge.
ACTIVITY CARDS. All students participating must purchase a Wood River Middle School (WRMS) Activity Card. Students must pay in full the WRMS Activity Card fee before issuance. It is strongly suggested that students purchase a WRMS Activity Card purchase the activity card at the beginning of the school year to take full advantage of attending all WRMS regular season athletic events free of charge.
ACTIVITY CARDS. Each Employee, spouse and dependent of high school age or older (still living at home or who is a full‐time student) shall be provided a complimentary ticket pass good for admission to all in‐ District school athletic activities. Use shall be limited to the Employee, spouse and immediate family, provided, however, the Employee or spouse must accompany any children not yet in high school who seek admission by use of a pass. The District shall provide each Employee with a pass near the beginning of each school year. Additional passes for family members falling under the above guidelines must be requested by the District Employee in writing to the Unit Office and the issuance of said passes is subject to verification by the District.

  • Promotional Examinations All promotional examinations shall be competitive and shall be conducted in the same manner as open examinations except that admission to a promotional examination shall be restricted to persons in the employ of the County who have served at least six months following regular appointment to the classified service and who meet the published requirements for the examination. The Commission shall determine whether an examination shall be held on a promotional or open basis. 1492 Disposition of Papers and Records Prescribed application forms of persons who fail to appear for the written test and examination records of candidates who fail to qualify in the written test shall be destroyed at any time after thirty days from the date of promulgation of the eligible list. Examination records of candidates who qualify in an examination shall be retained during the life of the eligible list or for 15 months, whichever is longer, and the examination records of each appointee shall be filed in his/her permanent personnel folder. Following the period during which competitors may inspect their examination papers, and after their ratings in each part of the examination have been transferred to examination records, examination materials such as question booklets, answer sheets and work papers may be destroyed, but general qualification appraisal sheets and applications shall be retained at least 15 months from the date of the promulgation of the eligible list. APPOINTMENT

  • Sports related devices, services and medications used to affect performance primarily in sports- related activities; all expenses related to physical conditioning programs such as athletic training, bodybuilding, exercise, fitness, flexibility, and diversion or general motivation.

  • Footwear Where an employee is required by the College or by legislation, in order to perform his/her duties, to acquire and wear protective footwear, the employee shall provide the College with proof of purchase by March 1 each year and the College shall reimburse such employee, on the first pay day in April in each year, up to a maximum of one hundred and fifty dollar ($150.00). In situations other than the foregoing, the College may, in its discretion, (which discretion shall not be unreasonably exercised) reimburse such expense where it is recommended by the health and safety committee constituted under the Occupational Health and Safety Act.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias. Procedures include but are not limited to: • Rapid Palatal Expansion (RPE); • Placement of component parts (e.g. brackets, bands); • Interceptive orthodontic treatment; • Comprehensive orthodontic treatment (during which orthodontic appliances are placed for active treatment and periodically adjusted); • Removable appliance therapy; and • Orthodontic retention (removal of appliances, construction and placement of retainers).
