An Employee must provide the Employer with confirmation from the adoption agency of the start of the placement.
An Employee must provide the Employer with written proof of the requirement to attend for jury service and an estimate of the duration of the absence from duty.
An Employee must provide. Xxxxx the Grocer Adelaide with the same notice of his or her resignation as specified in the table in clause 40.3 but without the requirement for an additional week’s notice if the Employee is over 45 years of age. Xxxxx the Grocer Adelaide may agree to a shorter period of notice if so requested by an Employee.

An Employee must provide. A DOCTOR'S STATEMENT SATISFACTORY TO THE COMPANY IF OFF WORK BECAUSE OF A CLAIMED INJURY OR ILLNESS, IF REQUESTED BY THE COMPANY. IT IS NOT THE INTENT OF THE COMPANY TO MAKE SUCH REQUEST UNLESS THE EMPLOYEE HAS AN ATTENDANCE PROBLEM OR THE EMPLOYEE IS OFF FOR ILLNESS OR INJURY FOR THREE (3) WORKING DAYS OR MORE OR THE COMPANY SUSPECTS ABUSE.
