ASSOCIATION DISABILITY, OPTICAL AND LIFE INSURANCE PROGRAMS. A. Each Unit member who is a member of the Los Angeles City Employees' Retirement System (LACERS) shall be enrolled in the Association Disability, Optical and Life Insurance Programs. Management will forward $32.00 bi-weekly to carriers designated by the Association for each Unit member who is on active payroll status and a member of LACERS. Such amount shall be allocated for the Association Disability Program, Optical Program and Life Insurance Program.

ASSOCIATION DISABILITY, OPTICAL AND LIFE INSURANCE PROGRAMS. Each unit member, who is a member of the City Employees' Retirement System (CERS) will be enrolled in the Association Disability, Optical and Life Insurance Programs. Management will forward thirty-two dollars ($32.00) bi-weekly to carriers designated by the Association for each employee in the Unit who is on active payroll status and a member of CERS. Such amount shall be allocated for the Association Disability Program, Optical Program and Life Insurance Program. Operative on the date a unit member became/becomes a member of Tier 5 of the Fire and Police Pension System, each unit member will be enrolled in the Association Optical and Life Insurance Program only. Management will forward twenty-five dollars ($25.00) bi- weekly to carriers designated by the Association for each employee in the Unit who is on active payroll status and a member of Tier 5. Such amount shall be allocated for the Association Optical Program, and Life Insurance Program. The Controller and Personnel Department will establish such controls over the disbursement of funds as they deem necessary. The Association agrees to indemnify and hold harmless the City against all claims, including costs of suits and reasonable attorney fees and/or other forms of liability arising from the implementation of the provisions of this Article.

The Controller and Personnel Department will establish such controls over the disbursement of funds, as they deem necessary. The Association agrees to indemnify and hold harmless the City against all claims, including costs of suits and reasonable attorney fees and/or other forms of liability arising from the implementation of the provisions of this Article.

  • Long Term Disability Insurance Plan The Employer shall provide a mutually acceptable long-term disability insurance plan, a copy of which shall appear in Appendix “A” – Long-Term Disability Insurance Plan. The plan shall provide post-probationary regular employees with salary continuation as per Appendix “A” until age sixty-five (65) in the event of a disability. The cost of the plan shall be borne by the Employer.

  • STATE DISABILITY INSURANCE (SDI) A. All employees covered by this Contract will be covered under the State Disability Insurance (SDI) benefit in lieu of a Non-Industrial Disability Insurance (NDI) and Enhanced Non-Industrial Disability Insurance (ENDI) benefit as follows:

  • Long Term Disability Insurance 250. The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Long Term Disability Plan The Welfare Plan will include a Long Term Disability Plan summarized in Appendix “2”.

  • DISABILITY INSURANCE PLAN Management shall expend for active employees of this unit who are members of LACERS the sum necessary to cover the cost of a basic disability insurance plan. Management shall also maintain a Supplemental Disability Insurance Plan, enrollment in which is at the discretion of each employee. The full cost of the Supplemental Disability Insurance Plan premiums shall be paid by the individual employees who enroll in the plan. The City's Joint Labor-Management Benefits Committee shall determine the benefits and provider of the plan

  • SICK LEAVE AND LONG-TERM DISABILITY (Articles 12.01 to 12.11 apply to full-time nurses only)

  • Workplace Safety Insurance Benefits (WSIB) Top Up Benefits If the employee is in a class of employees that, on August 31, 2012, was entitled to use unused sick leave credits for the purpose of topping up benefits received under the Workplace Safety and Insurance Act, 1997;

  • State Disability Insurance (“SDI”) 215. Employees in the bargaining unit(s) covered by this agreement shall be enrolled in the State Disability Insurance Program. The cost of SDI will be paid by the employee through payroll deduction at a rate established by the State of California Employment Development Department.

  • Non-Industrial Disability Insurance A. Non-Industrial Disability Insurance (NDI) is a program for State employees who become disabled due to nonwork-related disabilities as defined by Section 2626 of the Unemployment Insurance Code.

  • Long Term Disability (LTD 4.7.1 The school board shall cooperate in the administration of the LTD Plan. It is understood that administration means that the school board will co-operate with the enrolment and deduction of premiums and provide available necessary data to the insurer, upon request. The school board will remit premiums collected to the carrier on behalf of the teachers.
