Optical Program. An Optical Insurance Program will provide the following: 1. Employees (including DROP Participants) covered by this Agreement and their dependents will be covered by a Blue Cross/Blue Shield Vision Care Program known as Series A80, or its substantial equivalence.

Optical Program. Upon presentation of an original, itemized receipt including the patient’s name, date of service, total vision costs and payment in full, the City will reimburse 50% of the optical costs incurred by the employee for the employee, their legal spouse and eligible dependent children. (Dependent is defined as: family members currently listed on, or eligible to be on, your City of Port Huron BCBS healthcare plan.) Effective January 1, 2015, the optical reimbursement under this program will be increased to a maximum of $250.00 per employee with no dependents or $500.00 per family per year. Effective January 1, 2023, each employee, their legal spouse and eligible dependents will qualify for $300 reimbursement per person per calendar year.
Optical Program. Effective January 1, 1995, the City will provide an optical program for each permanent, full-time employee. The optical program will also be available to all family members listed as dependents on the employee=s hospitalization policy. The plan year is defined as the twelve (12) month period, January 1 through December 31. Effective January 1, 2003, the optical reimbursement under this program will be increased to a maximum of $150.00 per employee with no dependents or $300.00 per family per year. Effective January 1, 2015, the optical reimbursement under this program will be increased to a maximum of $250.00 per employee with no dependents or $500.00 per family per year.

Vision care improvement: $200 for eye glasses/contact lenses; $300 for bifocal glasses per 24 months; and $75 eye exam per 12- month period. Included are all eligible full-time employees and their eligible dependents (spouse and unmarried children under 23 years of age who live with the employee in a regular parent/child relationship). The extension of benefits to dependents shall be effective only after the employee has been continuously employed for a minimum of 60 days. 2. Full-time employees and eligible dependents, as defined above, shall be eligible for a maximum payment of $30 or the cost, whichever is less, of an eye examination by an ophthalmologist or an optometrist. 3. Each eligible employee and dependent may receive only one (1) payment for glasses per 24-month period and one (1) payment for examination per 12-month period. Proper affidavit and submission of receipts are required of the employee in order to receive payments.
