Consent to Use Personal Information. 9.1 The Employee acknowledges and agrees that the Company has the right to collect, use and disclose the terms and conditions of his employment and any other identifying personal information required to be disclosed for reporting or business purposes or otherwise by law, including:
(a) ensuring that he is paid for his services to the Company;
(b) administering any benefits to which he is or may become entitled to, including bonuses, medical, dental, disability and life insurance benefits, and/or annual bonuses and long-term incentive securities. This shall include the disclosure of his personal information to any insurance company and/or broker or to any entity that manages or administers the Company’s benefits on behalf of the Company, subject to applicable laws;
(c) compliance with any regulatory reporting and withholding requirements relating to his employment; and
(d) in the event of a sale or transfer of all or part of the shares or assets of the Company, disclosing to any potential acquiring organization solely for the purposes of determining the value of the Company and its assets and liabilities and to evaluate the Employee’s position in the Company. If the Employee’s information is disclosed to any potential acquiring organization, the Company will require the potential acquiring organization to agree to use the information solely for the purpose of evaluating the Company and to protect the privacy of Employee’s information in a manner that is consistent with any policy of the Company dealing with privacy that may be in effect from time to time and/or any applicable law that may be in effect from time to time.
9.2 The Employee may withdraw his consent provided herein at any time. The Employee acknowledges that if he withdraws his consent, his entitlement to certain employment benefits provided by the Company may be negatively affected and in the event of a sale of business, the acquiring organization may not be in a position to offer continued employment due to a lack of personal information on the Employee.
Consent to Use Personal Information. 1. When the Landlord agrees to use the Agent’s services, he agrees that the Agent may use information given, including information about the Landlord, for the purposes of performing the Agent’s obligations to the Landlord.
2. The Landlord agrees that the Agent may supply such information as is reasonably required to the Scheme. The Landlord agrees that the Scheme, or the government department responsible for the Scheme, may contact the Landlord from time to time to ask the Landlord to participate in surveys. Statements, notices (which includes changes to this agreement), disclosures and other communications (together called communications) will be sent to you in writing and will be mailed or delivered to you at the address which is maintained in our records for your account (except as set out below). Upon such enrollment by us or your Company in an electronic statement service we may also send communications to you electronically instead of by mail. You agree that it is your responsibility to access and retain copies of all electronic communications that we may provide to you. Statements and other disclosures on the American Express website will be posted for a minimum of 60 days. You agree not to dispute any electronic communication on the basis that it was not in writing or was not signed. All mailed communications will be deemed received 3 business days after the date of the mailing unless you actually receive it earlier or when received in the case of a communication delivered by hand. Consent to Use Personal Information. 20.1 The Employee hereby consents to the collection, processing and further processing of the Employee's Personal Information by the Group, for the purposes of securing and further facilitating the Employee's employment.
20.2 Without derogating from the generality of the aforementioned, the Employee consents to the Group's collection and processing of Personal Information pursuant the clause 20 insofar as Personal Information of the Employee is contained in relevant electronic communications.
20.3 The Employee is hereby notified of the purpose and reason for the collection and processing of such Personal Information.
20.4 The Employee undertakes to make available to the Company all necessary Personal Information required by the Company for the purpose of securing and further facilitating the Employee's employment.
20.5 Without limiting the generality of the aforegoing, the Employee absolves the Group from any liability in terms of POPI for failing to obtain the Employee's consent or to notify the Employee of the reason for the processing of any of the Employee's Personal Information.

By signing this form, I consent to the use of my personal information by United Food & Commercial Workers Canada Union, Local No. 401 for the purposes listed above, and I consent to the sharing of my personal information with third parties by the Union.
