COVID-19 Vaccination Requirements. The City of Los Angeles enacted (on an Urgency Basis) Administrative Code section 4.700 which imposes certain specified COVID-19-related requirements on all City of Los Angeles employees in furtherance of the City's goal of having a fully vaccinated workforce. (A copy of the Ordinance is attached.) Section 4.701(c)(4) relates to new contacts executed by the City on and after October 20, 2021. Accordingly, as a material term of this contract with LAWA, Contractor agrees that all employees of Contractor and/or persons working on Contractor's behalf who interact with LAWA or City employees, are assigned to work on LAWA or City property for the provision of services, and/or come into contact with the public during the course of work on behalf of LAWA or the City are to be "fully vaccinated" as that term is defined in Section 4.700(d).

COVID-19 Vaccination Requirements. Beginning January 3, 2022, the City of Portland will require proof of COVID-19 vaccination for anybody performing in- person work for at least 15 minutes at the City’s indoor facilities. Contractor must attest that its personnel are fully vaccinated for COVID-19 or have a medical or religious exception verified by their employer. This requirement and details may change. COVID-19 Vaccination Requirements. On August 25, 2021, the Board adopted the policy that all Vendor employees who have regular direct contact with CPS students must be fully vaccinated on or before October 15, 2021, or as amended by the Board. It will be the responsibility of each Vendor to certify to CPS that they comply with this vaccination requirement and that they maintain accurate personnel records to verify compliance. Vendor organizations will certify compliance by logging into the CPS Supplier Portal beginning August 30, 2021.
COVID-19 Vaccination Requirements. For all workers on District property, Architect shall comply with all applicable federal, state, and local laws, orders, and regulations regarding COVID-19, including but not limited to California Department of Public Health Officer’s Orders.   • Notification Requirements 1. If the Family Leave is foreseeable, the employee must provide the agency/department with thirty (30) calendar days notice of his or her intent to take Family Leave. 2.
