Vaccination Requirement. Effective November 15, 2021, all vendors who may visit any District school site or facility and/or who may come into contact with District students or staff must be fully vaccinated against COVID-19. Vaccination Requirement. Students must submit appropriate vaccination information and forms to FAMU Student Health Services to reside in University Housing. Failure of the student to submit the required vaccination/immunization record will result in cancellation of the agreement and the University Housing will not refund the advance payment.

After October 18, 2021 Contractor Staff who are reasonably likely or contractually obligated to engage in work while physically present at a building, facility, jobsite, project site, unit, or other defined area owned, leased, occupied by, or controlled by a State Agency, an operator of an Educational Setting, or an operator of a Health Care Setting as defined in the Proclamation must be fully vaccinated against COVID-19 unless they have been granted a valid disability or religious accommodation by Contractor. Contractor shall obtain a copy of, or visually observe proof of full vaccination against COVID-19 for all Staff who are subject to the vaccination requirement in the Governor’s Order. Contractor shall follow the requirements for granting disability and religious accommodations to Contractor’s Staff that apply to State Agencies under the Governor’s Order. Contractor and Contractor Staff shall provide proof of such vaccination or accommodation upon request by DSHS. Contractor shall cooperate with any investigation or inquiry DSHS makes into the employer’s compliance with these requirements, including by providing information and records upon request, except any information or records that the employer is prohibited by law from disclosing.

Students must comply with University vaccination requirements for residential Students by submitting appropriate vaccination documentation to USF Student Health Services before Department will complete a Room assignment. The Department will not refund the Application Fee if the Agreement is cancelled due to non-compliance with the vaccination requirements.

The immunization record can be found in the student health form. The College and University Student Vaccination Act provides that the University shall prohibit a student from residing in a dormitory or housing unit unless the student has received a one-time vaccination against meningococcal disease. If the student is a minor, the vaccination may only be administered with the consent of the student’s parent or guardian. 35 P.S. § 633.1

b. The University will notify an employee via email within five business days of submission if the proof of 09032021_AFT_MOA-COVID-vaccination is deemed to be inadequate or incomplete, including any specific defect in the material submitted.
c. In order to request an exemption from the vaccination requirement for religious reasons protected by Title VII of the Civil Rights Act, or due to a health condition protected by the Americans with Disabilities Act, an employee must complete and upload a COVID-19 Vaccination Waiver Form via Vax Check no later than October 1, 2021, at 5 pm.
i. No new applications for exemption will be accepted after October 1, 2021 at 5 pm.
ii. The University will review each application and will contact the employee within five business days of receipt to request any supporting documentation, or to provide notice that the application is complete as submitted.
iii. An application will be deemed to be complete the earlier of: the University providing notification to the employee that the application is complete; or, upon receipt and review of the supporting documents by the University, ten business days following the initial request from the University for supporting documentation.
iv. The University will provide a response to the employee within five business days of receipt of a completed application, including supporting materials.
v. If the University denies a request for exemption, the denial will include a written specification of defects in the application. Upon receipt of notice that an application for exemption has been denied, an employee may submit an amended application within ten business days. No more than one amended version of an application for exemption may be submitted.
vi. The term of approval for an exemption is up to 180 days.

Individuals are considered “fully vaccinated” two (2) weeks after receiving the second dose in a two-dose COVID-19 vaccine series, or two (2) weeks after one dose of the single dose COVID-19 vaccine. Renter should note and advise its Representatives who intend or are reasonably likely to enter FOX premises that they may be required to show proof of vaccination when they arrive at FOX premises (e.g., to a security guard or other FOX personnel). For unvaccinated Representatives, a COVID-19 test must be administered within 24 hours prior to entering FOX premises. For individuals entering the FOX Studio Lot or FOX Plaza in Los Angeles, CA, as of the date of this Agreement, FOX can accommodate testing them at its cost.
