Credentialling. Cooperate with the Manager and the Quality Committee in establishing, criteria, policies and procedures for purposes of credentialling all of the LLC's physicians. Such credentialling criteria, policies and procedures shall be established in accordance with all community medical practice standards, all applicable laws, and the minimum participation criteria developed by the Manager and the Quality Committee. All of the LLC's physicians shall be required to comply with such credentialling criteria. All decisions with respect to credentialling shall be reached as follows:

Credentialling. 17.1 It is a term of this Agreement that Full Time Specialist’s are and at all times will be required to be credentialled for the procedures/operations which they undertake by the relevant Program Medical Director, appointed for this purpose by the Chief Executive Officer of Southern Health or his/her nominee, for the period that the Full Time Specialist provides medical services to Southern Health and its patients, under any arrangement between the parties, in accordance with any requirements prescribed by Southern Health. 17.2 It shall be a condition of the Full Time Specialist to continue to provide services to Southern Health that such credentialling must be both documented and reviewed annually by the relevant Program Medical Director.

RPR must include a component of external review that is by peers external to a doctor’s usual practice setting. • a pattern of practice over a period of time that suggests the doctor’s practice of medicine may not meet the required standard of competence; or • a single incident that demonstrates a significant departure from accepted standards of medical practice; or • recognised poor performance where local interventions have failed – this does not exclude notification of serious concerns where internal review or audit is inaccessible or unavailable to the person with the concern; or criminal offending; or • professional isolation with declining standards that become apparent. • an individual patient may be seriously harmed by the doctor; or • the doctor may pose a continued threat to more than one patient and as such the harm is collectively considered ‘serious’; or • there is sufficient evidence to suggest that the alleged criminal offending is of such a nature that the doctor poses a risk of harm to one or more members of the public.

Where a program currently exists and meets the criteria listed in this agreement, such a program shall be deemed to satisfy the provisions of this article. This awareness program shall initially be for all employees and shall be scheduled at least once annually for all new employees to attend. b. The awareness program shall include but not be limited to: i. the definitions of harassment and sexual harassment as outlined in this Agreement; ii. understanding situations that are not harassment or sexual harassment, including the exercise of an employer's managerial and/or supervisory rights and responsibilities; iii. developing an awareness of behaviour that is illegal and/or inappropriate; iv. outlining strategies to prevent harassment and sexual harassment; v. a review of the resolution of harassment and sexual harassment as outlined in this Agreement; vi. understanding malicious complaints and the consequences of such; vii. outlining any Board policy for dealing with harassment and sexual harassment; viii. outlining laws dealing with harassment and sexual harassment which apply to employees in B.C.

  • Medical Verification The Town may require medical verification of an employee’s absence if the Town perceives the employee is abusing sick leave or has used an excessive amount of sick leave. The Town may require medical verification of an employee’s absence to verify that the employee is able to return to work with or without restrictions.

  • Professional Conferences Professional conferences are those conferences sponsored by educational organizations, institutions of higher learning, and government and industry concerning teaching, learning and educational research. When a full-time or part-time faculty member’s attendance at a professional conference conflicts with his/her regular assignment, the faculty member must submit a request for attendance to the appropriate Xxxx, Director, or Supervisor. If granted, the attendance at the conference will be considered leave with pay, and a follow-up report of the conference shall be submitted to the appropriate Supervisor.

  • Training Programs All employees shall successfully complete all necessary training prior to being assigned work (e.g., all employees will complete health and safety training prior to being assigned to task). Nothing in this Article or provision shall constitute a waiver of either party’s bargaining obligations or defenses. The Employer still has an obligation to notify and bargain changes in terms and conditions of employment with the exclusive representative.
