Dependent Premiums. For those employees with dependents enrolled in the group's health insurance plan, the Employer agrees to pay up to three hundred fifty dollars ($350.00) per month towards the medical, dental and optical premiums for the employee's spouse and dependent children qualified to participate in the respective plans. The Employer contribution of $350.00 per month towards dependent insurance premiums shall take effect upon ratification of this Agreement and shall continue throughout the term of this Agreement. If there are any health insurance premium increases in any year(s) of this Agreement, CHMI Management and the Union agree to re-open this provision.

Dependent Premiums. 23 Full- or part-time employees may obtain dependent coverage under either plan 24 by paying the following percentages of the insurance premium. 26 For employees with less than twenty thousand eight hundred (20,800) hours of 27 service, dependent coverage may be obtained by paying thirty percent (30%) of the 28 insurance premium. Employees with more than twenty thousand eight hundred (20,800) 29 hours (but less than thirty one thousand two hundred (31,200) hours) of service may 30 obtain dependent coverage by paying twenty-five percent (25%) of the insurance 31 premium. Employees with more than thirty one thousand two hundred (31,200) hours 1 (but less than forty-one thousand six hundred (41,600) hours) of service may obtain 2 dependent coverage by paying twenty percent (20%) of the insurance premium. 3 Employees with more than forty-one thousand six hundred (41,600) hours of service 5 premium. Premium costs will be adjusted based on seniority hours in January and July 6 of each year.

  • Shift Premiums (a) Evening/Night Premium Employees working an evening or night shift where the major portion of hours worked fall within the hours of 1500 and 0800 hours, shall be paid at their regular hourly rate plus a shift premium of $2.10 ($2.75 effective July 1, 2015) per hour for all hours worked.

  • Benefit Premiums The Employer shall continue to pay its portion of insured benefit premiums, provided employees continue to pay their portion, as follows:

  • Shift Premium Effective July 1, 2020, an employee shall be paid a shift premium of ninety-five cents (95c/ ) per hour for each hour worked between 1500 hours and 0700 hours provided that such hours exceed two (2) hours if worked in conjunction with the day shift.

  • Vision Coverage A fully employee paid vision benefit will be available beginning January 1, 2021 subject to agreement by the subcommittee of the Joint Labor Management Insurance Committee to the benefit set determined through the state’s Request for Proposal (RFP) process.

  • Termination Costs If a Party elects to terminate this Agreement pursuant to Article 2.3.1 above, the terminating Party shall pay all costs incurred (including any cancellation costs relating to orders or contracts for Attachment Facilities and equipment) or charges assessed by the other Parties, as of the date of the other Parties’ receipt of such notice of termination, that are the responsibility of the terminating Party under this Agreement. In the event of termination by a Party, all Parties shall use commercially Reasonable Efforts to mitigate the costs, damages and charges arising as a consequence of termination. Upon termination of this Agreement, unless otherwise ordered or approved by FERC:
