Benefit Premiums. Employees on unpaid leave will be responsible for the entire cost of benefit premiums. Employer contributions shall be governed by applicable rules, regulations, or Florida law.
Benefit Premiums. The Employer shall continue to pay its portion of insured benefit premiums, provided employees continue to pay their portion as follows:
a. during the calendar month in which a lay off occurs;
b. during the first four (4) consecutive weeks of an authorized leave of absence without pay;
c. while the employee is off due to illness including the period when receiving sick leave benefits paid by the Employer, up to a maximum of three (3) months;
d. while in receipt of compensation under the Workplace Safety & Insurance Board as a result of an injury sustained during employment with the Employer, for up to eighteen (18) months;
e. while on maternity/paternal leave, for the period it is required to pay benefit premiums, in line with government legislation. Employees on any leave of absence and following the periods set out above may continue benefit coverage for a period of up to six (6) months providing they make arrangement with the Employer to pay the Employer and employees portion of all benefit premiums to the Employer by the 15th of the month on which the premium is due.
Benefit Premiums. The College will pay 100% of the premium of the benefits described in 27.02 for regular full-time employees. For regular part-time employees who are eligible to receive these benefits, the College will pay the proportion of the premiums that is the same as the employee's work assignment. All employees on general leave of absence, deferred salary or full-time Union and public duties leave in excess of one (1) month will be responsible for paying 100% of the premiums in advance.
Benefit Premiums. The College will continue to pay its portion of benefit premiums while an employee is off on approved sick leave.
Benefit Premiums a) (i) Subject to the Resident Physician or legal designate providing written confirmation to AHS of application for long term disability (LTD) benefits, the supplementary health and dental plan premiums, inclusive of the Resident Physician’s twenty-five (25%) share, shall be waived.

a. during the calendar month in which a layoff or leave of absence occurs;
b. while the employee is off due to illness including the period when receiving sick leave benefits as defined in Article 15.01 for up to eighteen (18) calendar months from the date of the absence;

a. during the month in which the employee’s leave of absence without pay commences;
b. while the employee is off due to illness, including the period when receiving paid sick leave benefits paid by the Employer, up to a maximum of three (3) calendar months;
c. while in receipt of compensation from the Workplace Safety and Insurance Board as a result of an injury sustained during employment with the Employer, for up to twelve (12) months;
d. while on maternity/parenting leave, for the period it is required to pay benefit premiums, in line with government legislation. Employees on leave of absence and following the periods set out above may continue benefit coverage until such time as they lose their seniority, providing they make arrangements with the Employer to pay the Employer and employee portion of all benefit premiums to the Employer by the 15th of the month on which the premium is due.
Benefit Premiums. The Employer shall continue to pay its portion of insured benefit premiums, provided employees continue to pay their portion, as follows:
a. during the month in which the employee’s leave of absence without pay commences;
b. while the employee is off due to, injury, disability, or illness, including the period when receiving paid sick leave benefits paid by the Employer, up to a maximum of six (6) calendar months;
c. while in receipt of compensation from the Workplace Safety and Insurance Board as a result of an injury sustained during employment with the Employer, for up to twelve (12) months;
d. while on pregnancy and parental leave; and
e. for any other period it is required to pay benefit premiums, in line with government legislation.
Benefit Premiums. The School Association shall continue to pay for insured benefit premiums, provided employees continue to pay their portion (where applicable) under the following conditions:
a. during the first four (4) consecutive weeks of a Board-authorized leave of absence without pay;
b. while the employee is off due to illness or injury including the period when receiving sick leave benefits paid by the School Association;
c. while on pregnancy, parental, personal emergency, family medical, or declared emergency leave, for the period it is required by legislation to pay benefit premiums. Employees on any approved leave of absence may continue benefit coverage, provided they make arrangements with the Director of Finance to pay both the School Associations and Employee’s portion of benefit premiums to the School Association by the 15th of the month in which the premium is due and provided they are eligible to participate in the benefit plan. If payment is not received by the Director of Finance by the 15th of any month, coverage will be discontinued.
a. The School Association is responsible at all times for the enrolment and the proper remittance and payment of premiums to the insurance carrier(s). ACTSS shall be supplied with a copy of the insurance policy(s).
b. All employees covered by the insurances shall be supplied with a copy of a booklet as provided by the insurance company, outlining the coverage to which they are entitled.
Benefit Premiums a) The employee may choose to pre-pay both the employee and employer portions of the applicable employee benefit premiums before the leave commences or may request that all benefit plans be held open pending resumption of employment.
b) An employee will be retroactively reimbursed the employee’s portion of the benefit premiums, which the employee prepaid, for the medically related portion of the leave.
c) Upon return from parenting leave an employee shall be entitled to purchase prior service into the Local Authorities Pension Plan.
