Direct Mail Notice. Beginning not later than December 2, 2016, the Settlement Notice Administrator shall send the Direct Mail Notice, substantially in the form attached hereto as Exhibit 6, by U.S. Mail, proper postage prepaid, to the current and former registered owners of Subject Vehicles, as identified by data to be forwarded to the Settlement Notice Administrator by X.X. Xxxx & Co. The Direct Mail Notice shall inform potential Class Members on how to obtain the Long Form Notice via the settlement website, via regular mail or via a toll-free telephone number, pursuant to Sections IV(E) and (F), below. In addition, the Settlement Notice Administrator shall: (a) re- mail any notices returned by the United States Postal Service with a forwarding address no later than the deadline found in the Preliminary Approval Order; (b) by itself or using one or more address research firms, as soon as practicable following receipt of any returned notices that do not include a forwarding address, research such returned mail for better addresses and promptly mail copies of the applicable notice to any better addresses so found.

Direct Mail Notice. No later than sixty (60) days after the entry of Preliminary Approval, the Settlement Administrator shall send Notice and an accompanying Claim Form (with return postage pre-paid via postcard), substantially in the form attached as Exhibit C, via First Class Mail through a postcard to the last known physical address of those Settlement Class Members for whom a valid email address is not found on the Class List.
Direct Mail Notice. No later than the Notice Date, the Settlement Administrator shall send Notice and accompanying Claim Form (with return postage pre- paid via postcard), substantially in the form attached as Exhibit B, via First Class Mail through a postcard to those Settlement Class Members who appear on the Class List with a valid U.S. Mail address or to each physical address reasonably obtainable.
Direct Mail Notice. The Claims Administrator shall provide for Direct Mail Notice to Settlement Class Members. This Direct Mail Notice shall be in a manner agreed to by the Parties and approved by the Court, and will be substantially in the form of the Direct Mail Notice attached as Exhibit A. The Direct Mail Notice shall include the Claim Form and refer possible Settlement Class Members to the Website Notice on the Settlement Website and provide a toll-free number for obtaining details regarding the Settlement and the Claim Form. In the event that any Direct Mail Notice is returned to the Claims Administrator, the Claims Administrator shall resend by First-Class Mail, postage prepaid, the Class Notice to the forwarding address, if one is provided by the United States Post Office. For any other Direct Mail Notices that are returned as undeliverable, the Claims Administrator shall conduct a search for alternative or updated addresses, and upon identification of an evidently valid alternative address, will resend the Direct Mail Notice to the Settlement Class Member. In the event that any Direct Mail Notice is returned as undeliverable a second time, no additional mailing shall be required or performed by the Claims Administrator or the Parties. The Parties and their counsel shall not issue additional or supplemental notice absent the consent of all Parties and prior Court approval.
Direct Mail Notice. The Court approves the Direct Mail Notice without material alteration from Exhibit A to the Settlement Agreement. Within forty-five (45) days of entry of the Order of Preliminary Approval, or such time as the Parties may agree, the Claims Administrator shall print and cause to be mailed by First-Class Mail, postage prepaid, the Direct Mail Notice. The Direct Mail Notice will be provided to all health care providers who are identified as potential Settlement Class Members, at their last known address of record, as determined by a reasonable search of the bill review database provided to the GEICO COMPANIES by their medical bill review vendors in Florida during the Class Period. Addresses of potential Settlement Class Members identified by the GEICO COMPANIES will be run through the National Change of Address database (the “NCOA”) prior to mailing. The Claims Administrator shall check and update all class notice addresses pursuant to an NCOA review prior to mailing.
Direct Mail Notice. On the Notice Date, the Settlement Administrator shall send the Direct-Mail Notice. The Settlement Administrator shall not be required to send such Direct-Mail Notice to any Class Member whose last known street address, as updated through the National Change of Address registry, is determined to be undeliverable pursuant to one of the following mailing codes: F (foreign move, no new address available), G (postal box closed, no new address available), or K (move, left no forwarding address).

In addition to the Direct Mail Notice, attached hereto as Exhibit B, Toyota reserves the right to include as part of the Direct Mail Notice, a Customer Letter from Toyota – the content of which shall be agreed to by the Parties and approved by the Court. The Customer Letter shall provide Class Members with specific direction and information on when and how to receive the benefits of the Customer Confidence Program.

Direct Mail Notice. 1. Consistent with the timeline specified in the Preliminary Approval Order, the Settlement Notice Administrator shall send the Direct Mail Notice, substantially in the form attached hereto as Exhibit 4, by e-mail where a valid Class Member e-mail address is available, or U.S. Mail, proper postage prepaid, to the current and former registered owners of Subject Vehicles, as identified by data to be forwarded to the Settlement Notice Administrator by IHS Markit, now part of S&P Global. The Direct Mail Notice shall inform those persons of how to obtain the Long Form Notice via the settlement website, via regular mail or via a toll-free telephone number, pursuant to Sections IV.D. through IV.F., below. In addition, the Settlement Notice Administrator shall: (a) send a Direct Mail Notice to any Class Member who was initially sent a Direct Mail Notice by e-mail and was subsequently determined to be undeliverable; (b) re-mail any notices returned by the United States Postal Service with a forwarding address; (c) by itself or using one or more address research firms, as soon as practicable following receipt of any returned notices that do not include a forwarding address, research such returned mail for better addresses and promptly mail copies of the applicable notice to any updated addresses so found. 2. The QR code associated with the Direct Mail Notice shall remain active and the link associated with the QR code shall be maintained in proper working order by the Settlement Notice Administrator for the duration of the Customer Support Program.
Direct Mail Notice. For those members of the Settlement Class for whom no email address is available or for whom email notice was returned as undeliverable, but a Valid Mailing Address is available, the Settlement Administrator shall mail the direct notice in substantially the form attached hereto as Exhibit #3 to the Valid Mailing Address for each such Settlement Class Member. For any initial direct mail notice that is returned as undeliverable within twenty-one (21) days after mailing, the Settlement Administrator shall attempt to locate a new address through an address search or any other reasonably available means. If a new address is located, the Settlement Administrator shall promptly re-mail the initial notice. If, after a second mailing, the notice is again returned, no further efforts need be taken by the Settlement Administrator to send the direct mail notice.
