Early Warning System. The Calderdale Older People’s Commissioning Group also provides the opportunity for monitoring of activity and resources across health and social care, including; usage of home care, demand and capacity within non-acute beds, and demand and capacity within community based services. The direct benefit locally of this target is a reduction of almost 3,000 bed days for people aged 65+. Indirect benefits would be improved quality of life and patient experience for those people who would otherwise have been admitted to hospital for an acute episode of illness The Long Term Conditions Programme is being developed with a similar multi-agency approach and supports a reduction in EBDs, particularly in relation to COPD and Heart Failure. Both conditions contributed significantly towards the current level of EBDs, and usage of reward funding would significantly contribute towards the ability of local organisations to reduce EBD usage. The Falls Prevention Project also supports reduction in EBD with multi-agency working at its centre Pump-priming funds would be used to support development of Chronic Obstructive Pulmonary Disease and Heart Failure services (£50k each –total £100k) alongside all the above initiatives The inclusion of the EBD target gives a clear mandate to local services to work much more collaboratively to reduce current levels of hospitalisation for those aged 65+. It also ensures that delivery and monitoring of target is clearly set within organisational objectives of a broader range of key partners. The LAA is an ideal mechanism to support this process.

Early Warning System. Principle: The Lakewood Police Department recognizes that to maintain an intricate environment of teamwork and accountability, systems must be in place to support members when facets of the profession challenge them and they may need additional assistance or intervention in meeting department expectations. The purpose of the early warning system is to provide early, identifiable negative performance indicators that will assist supervisors and administrators in identifying an employee whose performance may need review with potential corrective action. This system is a tool for monitoring performance and is NOT a disciplinary process and the result of the Early Warning System (EWS) is not considered disciplinary action. The EWS Board consists of the PSS Sergeant, the Peer Support Lieutenant and the Assistant Chief. The employee’s first level supervisor works directly with the board once convened.
