Employee Authorization. By signing below, I hereby authorize my employer, Montefiore to deposit my net pay directly into my checking or savings account each payday. If any monies to which I am not entitled are deposited into my account for any reason, including as the result of Montefiore’s error I authorize Montefiore to direct the bank to return such funds directly to Montefiore in the full amount of the improper payment. This authorization allows Montefiore to direct my bank to return the funds at the time the overpayment is discovered, regardless of when the funds were improperly deposited into my account. I agree that this authorization will remain in effect until I provide my employer with written cancellation to terminate this service. I understand that 4 weeks must be allowed for implementation and any changes in direct deposit. SIGNATURE DATE
Employee Authorization. No deductions will be made from the wages of any employee until the Employer has received a signed copy of the voluntary written authorization for such deductions.
Employee Authorization. An employee shall, as a condition of continued employment, complete an authorization form providing for the deduction from the employee's monthly wages or salary the amount of the regular monthly dues payable to the Union by a member of the Union.

The Company agrees to deduct from the pay of each employee voluntary contributions to a Political Action Committee provided that each such employee executes or has executed an "Authorization for Assignment and Check-Off of Contributions to a Political Action Committee" form and provided further that such authorization has not been revoked. Deductions shall be made only in accordance with the provisions of and in the amounts designated in said form, together with the provisions of this Section of the Agreement. A properly executed copy of "Authorization for Assignment and Check-Off of Contributions to a Political Action Committee" for each employee for whom voluntary contributions to a Political Action Committee are to be deducted hereunder, shall be delivered to the Company before any such deductions are made. All deductions shall be made pursuant to such properly executed forms for so long as they remain in effect. Such deductions shall be made from the employee's regular pay each pay cycle that the authorization remains in effect.

Employee Authorization. Employee hereby authorizes NCI, without further notice to Employee, to withhold either from Employee’s final pay, accrued bonus, any final expense reimbursement due Employee, or any accrued vacation amount, such amounts sufficient to satisfy the repayment obligation described in Paragraph 2 of this Agreement.
Employee Authorization. TVA makes payroll deductions for union dues and initiation fees from an employee's pay only upon receipt of an employee's authorization. Such required authorization form is considered a part of this supplementary agreement. So long as the employee remains in a position within the jurisdiction of the EA and the employee has authorized dues deductions, the employee's authorization shall be irrevocable and continue in full force and effect: (1) for a period of one year from the effective date of the employee's authorization, and if not revoked as described below, each annual renewal thereafter; (2) until the employee's termination or transfer to a position not covered by the EA Articles of Agreement; or (3) until the expiration of this collective bargaining agreement; whichever occurs first. An authorization is automatically renewed annually unless TVA receives a written notice of revocation, with a copy to the union, within the 30-day period immediately preceding the annual anniversary date of the employee's authorization. Revocations received by TVA are effective for the month following the annual anniversary date, or as soon as practicable thereafter. To reinstate payroll deductions, an employee must execute a new authorization. In the event an employee transfers to another represented salary policy position outside the bargaining unit represented by the union to which the employee's current dues authorization is made, the employee's authorization is not automatically revoked, but can be revoked at any time after the transfer by giving written notice to TVA with a copy to the union; such revocations are effective for the month following TVA's receipt of the written notice, or as soon as practicable thereafter. S–15 Medical/Dental Insurance Plan A. Coverage TVA and the EA provide a medical/dental benefits plan for employees on a voluntary basis. Coverage provisions of the plan are negotiated between TVA and the EA. TVA reserves the right to determine the financial arrangements and entity by which the benefits of the plan are provided. All annual employees, except part-time employees working less than 16 hours per week, may enroll in this plan regardless of age and without health requirements if they apply within 30 days of employment or transfer to an eligible position, or during any open enrollment period.
Employee Authorization. Each employee hereby authorizes the Union and the Employer, without recourse, to rely on and to honor certificates of the local Union regarding the amounts to be deducted and the legality of deducting such Union dues, assessments, or fees.
Employee Authorization. Employee authorization shall be in writing in the form set forth by the Board.
Employee Authorization. The Employer agrees to deduct OPBA membership dues once each month from the pay of any eligible employee in the bargaining unit upon receiving written authorization signed individually and voluntarily by the employee. The signed payroll deduction form must be presented to the Employer by the employee or the employee’s designee. Upon receipt of the proper authorization, the Employer will deduct OPBA dues from the payroll check for the next pay period in which dues are normally deducted following the pay period in which the authorization was received by the Employer.
