Form Letter. The Employer agrees to provide each new employee at the time of employment with a form letter, outlining to the employee his/her responsibilities in regard to Union membership and outlining the provisions of Article 5.4 of this Agreement; and to provide the Union, in writing, with the name and address of each employee to whom the letter was presented along with the employee's date of hire. The Union shall bear the expense of printing the letter, the contents of which to be such that it is acceptable to the Employer. The Employer further agrees to provide the Union, once a month, with a list containing the names of all employees who have terminated their employment during the previous month.

Form Letter. TO: Skyservice Airlines Inc. Date: You are hereby directed to pay all monthly compensation allowable to me, from Skyservice Airlines Inc. under the terms of “Missing, Hijacking, Internment, Hostage or Prisoner of War” Article of our Agreement to those designated as follows: (Name and address) as long as living and thereafter to: (name and address) (Name and address) as long as living. The balance, if any, and any amounts accruing after the death of all persons named in the above designations shall be held for me. In the event of my death before receipt thereof, said balance and amounts accruing shall be paid to the legal representative of my estate. The foregoing direction may be modified from time to time by letter signed by the undersigned, and any such modifications shall become effective upon receipt of such letter by you. I agree to indemnify and hold the Company harmless from any claims made relating to payments made by the Company pursuant to this direction and further. Each new employee will have the opportunity to have a meeting with a Shop Xxxxxxx (or Business Agent) in order to familiarize them with the Collective Bargaining Agreement and procedures contained in it. The introductory meeting shall be considered as time worked and will not exceed fifteen (75) minutes.
