Group Long-Term Disability Insurance.
12.01 The Division shall administer, in accordance with the Memorandum of Agreement between the parties, The Manitoba Teachers' Society Disability Benefits Plan No. 51155 for participating employees. The premium for the plan shall be paid by each employee participating in the plan and shall be deducted monthly from salary payments of those employees and timely remitted to the plan.
Group Long-Term Disability Insurance. Monthly payments will be made following the completion of the required period of continuous total disability (Benefit Waiting Period) resulting from accident or sickness that occurs either at or away from work. The Board of Education shall pay the same share of the cost of the premium for this insurance as the Board pays for health insurance. The monthly maximum benefit will be $2,000.
Group Long-Term Disability Insurance. District participation - 100% of the premium for each participating employee.
Group Long-Term Disability Insurance. ELIGIBILITY:
Group Long-Term Disability Insurance. The School District will pay each month 100 percent of the current premium for income protection insurance for each full-time health associate. The income protection plan shall include the following:
1. Benefits begin after ninety (90) calendar days of total disability.
2. The monthly income benefit shall be 66-2/3 percent of basic monthly earnings (exclusive of any additional compensation from this district or any other source).
Group Long-Term Disability Insurance. All Fire Fighters of the Municipality, from commencement of employment, must be covered by a Group Long Term Disability Insurance Plan. The benefit amount is 75% of gross earnings (maximum of $4,500./per month) after a seventeen (17) week qualifying period. The cost of this insurance plan is 100% paid by the Municipality. New employees may be required to produce satisfactory evidence of insurability. The Municipality agrees not to change the carrier without prior consultation with the Association.
Group Long-Term Disability Insurance. The benefit shall be provided in accordance with the terms and provisions of the Anthem Life Insurance group policy.
C. Part-time teachers shall have these benefits in proportion to their work week as determined by the Administration.
D. All insurance coverage will be offered for retirees to age 65 at their expense and subject to carrier approval.
E. At its expense, the Board shall provide insurance coverage as set forth in this Article for each teacher covered by this Agreement. The Board may provide such insurance either through the carrier(s) listed herein or through alternate carriers provided that the coverage, benefits, and administration of claims is equal to or better than that provided by the carrier(s) listed herein. In terms of the administration and payment of claims, any carrier that normally provides payment of claims within 30 days from the date that the claim is filed with the carrier(s) or its agent(s) shall be deemed to be equal. The President of the Association shall be notified in writing within 30 days of any intention to change carriers and shall have a reasonable opportunity to review the proposed changes. Should the Board and the Association disagree that the changes proposed will not result in coverage, benefits and administration of claims that are equal, then the Association, within 30 days of the notification to its President of the proposed change, may file a grievance. Such grievance shall immediately proceed to the Superintendent pursuant to the grievance procedure of Article 3 of this Agreement. In addition, if the Board and the Association disagree at any time that the changes implemented did not result in equal coverage, benefits, and administration, the Association may, within 30 days of its discovery, file a gnevance.
F. Subject to law, including the rules and regulations of the Internal Revenue Service, the Board shall maintain a "Section 125" salary reduction agreement which shall be designed to permit exclusion from taxable income of the employee's share of health insurance
Group Long-Term Disability Insurance. (currently Canada Life) - UWO pays the premium costs.
Group Long-Term Disability Insurance. 100% of the monthly premium cost for group long term disability insurance (S.E.T. Policy #G 5050.5 or equivalent). This policy shall provide 90 calendar day waiting period, 70% benefit level. The covered employee shall be responsible for the first 30 calendar days of illness/disability, with the Board as employer providing additional sick days for coverage through the 89th calendar day.
