Guidelines for Borrowing and Use. To borrow a Mobile Beacon hotspot, patrons must be at least 18 years old. ● Hotspots may be checked out to adult cards that are accepted by the Coloma Public Library and are in good standing. ● Only one hotspot may be checked out per address. ● Patrons must present a current id at the time of checkout (name on ID and library account must match). ● At the time of checkout, a staff member will verify that the hotspot is functioning properly. ● Hotspots may be borrowed for two weeks. Renewals are permitted when there are no pending hold requests. ● The Library is not responsible for information accessed while using the hotspot; for personal information shared over the Internet; or for computer viruses that may be transferred to user storage devices. ● The Library is not responsible for damage to personal property due to the use of the hotspot. ● Hotspots must be returned inside the Library rather than the book drop. Staff will verify that the hotspot is functioning properly before checking it in from the patron’s account.

Guidelines for Borrowing and Use. WARNING: Do not look at the sun with the telescope; doing so can cause blindness and damage the telescope. The library is not responsible for any damages a patron inflicts upon oneself or the telescope. • Only borrowers 18 years and older may check out the telescope. • Borrowers must present a valid Louisville Free Public Library card and be in good standing at the time of check out. • Check out period is one (1) week, no renewals. For each day overdue, a $10.00 charge will be added to my library account. If not returned within 10 days of being overdue, replacement costs outlined below for the telescope will be charged to my account. Replacement costs also will be applied for any damage. • Borrower may only check out one telescope at a time and must be returned by appointment at the same location it was checked out from. I will not clean the telescope, lenses or peripheral equipment as it may permanently damage the telescope (the telescope is disinfected prior to checkout by trained library staff).

These items are not available through Curbside- Pickup. • A Valid Humboldt County Library card with current address and no outstanding fines above $5.00 is required to borrow a Mobile Device. • A Valid government issued ID with picture and current address must be presented. Library personnel will make a photocopy and retain with this agreement for one year, prior to Mobile Device checkout. If the address on the ID and account do not match, you will be asked to validate your account by bringing proof of address. • Borrowers must be at least 18 years old. Xxxxxxxx’s ages 13 to 17 can check-out mobile devices for in- library use with an Agreement signed by the parent/guardian on file prior to their first check-out.

Children under age 18 must be supervised by an adult while using the telescope.  Telescope must be transported in a vehicle, and secured safely with the three pillows in the provided tote with the eyepiece and EZ-Finder positioned up towards the lid so it will not be damaged. Loan Rules and Duration  1 week (7 day) loan—NO RENEWAL  Reserve online through our TixKeeper System (the one used for museum passes) or call the library to reserve.  The telescope will only be available for pick up on the day of your reservation (no early pick-up).  The telescope will be checked out from the Information/Reference Desk in the Adult Library.  The telescope must be returned only to a librarian at the Information/Reference Desk in the Adult Library.  The telescope may only be picked up by the person with the reservation. Do not loan to others.  Fines are $5 per day. If the telescope is not returned within two weeks of its due date, you will be charged the $375 replacement cost. The Fitchburg Police Department may be notified.

Fines & Liability:  The overdue fine is $10.00 per day.  The borrower is responsible for costs associated with damage or loss of E-reader and/or peripherals due to neglect or abuse.  The replacement cost for an E-reader is $120.  The replacement cost for the power adapter is $20.00.  The replacement cost for the E-reader cover is $20.00.   • ORDERING REQUIREMENTS Eligible Purchasers shall order Goods and/or Services from this Master Contract, consistent with the terms hereof and by using any ordering mechanism agreeable both to Contractor and Purchaser but including, at a minimum, a purchase order. When practicable, Contractor and Purchaser also shall use telephone orders, email orders, web-based orders, and similar procurement methods (collectively “Purchaser Order”). All Purchase Orders must reference the Master Contract number. The terms of this Master Contract shall apply to any Purchase Order and, in the event of any conflict, the terms of this Master Contract shall prevail. Notwithstanding any provision to the contrary, in no event shall any ‘click-agreement,’ software or web-based application terms and conditions, or any other agreement modify the terms and conditions of this Master Contract.
