b. To have my body temperature taken with a non-contact forehead thermometer as a condition to entering the Project premises and will immediately exit the Project premises if my body temperature is 100.4 degrees or higher;
c. To comply with all project management, sanitation and cleanliness and “social distancing” rules, policies and guidelines established by Rebuilding Together that are applicable to the Project, including with respect to hand washing, wearing a protective face mask and gloves, and using my best efforts to maintain an appropriate distance from others; and
d. That I (a) am participating in the Project at my own risk and may be exposed to persons carrying or who exhibit symptoms of the COVID-19 virus, (b) have voluntarily assumed such risk and (c) have waived and released any and all claims related to the foregoing under paragraph 4 of this Release.
I agree. To indemnify and hold harmless “GSSN”, and any of the “GSSN” directors, employees, or agents, against any cause and all claims, demands, loss, damage, judgments, and expenses, including attorney’s fees, court costs, and any other legal expenses arising out of the use of “GSSN” facilities and amenities.

I agree. (a) to immediately notify the Organisation or the Department (as appropriate) if I become aware of a breach or possible breach of any of these obligations, whether by me or by any other person;
(b) that any third person who undertakes work on my behalf who is required to deal with Personal Information for the purposes of this program is made aware of these obligations.

I agree. That I will comply with all instructions on the use of tanning devices and that I am using these services at my own risk. (2) I agree to hold harmless all Fitness World affiliates, its employees and agents for any injury to person or property in any way by the use of its premises and services. In addition, Fitness World is not liable for the loss or theft of any personal property. Each person is responsible for safeguarding his or her own property.

Additional telecommuting equipment or service expenses (such as an additional telephone line, ISP connection fees or software), if any, must be approved in advance. Any approved equipment will be paid for or reimbursed and is considered county property.
I agree. To maintain the confidentiality of all county information and documents, prevent unauthorized access to any county system or information, and dispose of work-related documents in a manner that will not jeopardize the interests of the county. I will adhere to applicable King County policies relating to remote access and use of VPN connections while using my personally owned computer or laptop for telecommuting purposes. King County has the right to monitor all information generated and actions performed using remote access technology while I am telecommuting. I will ensure that my personally owned computer, netbook, laptop, or other similar device has anti-virus software, (either provided by King County or of equivalent quality) installed and configured to properly receive malware signature file updates on a daily basis. I am responsible for all activity originating from my account credentials (username and password).
