Hazard Premium Pay. Mental Health Crisis Intervention Pay The City of Berkeley (City) and Berkeley Police Association (Association) agree that the parties may agree under this MOU provision that the City will provide Hazardous Premium Pay for particular hazardous duties and activities. At present, the City and Association have agreed that, effective the first full pay period after Council approval of this MOU, all sworn employees shall receive a Hazard Premium Pay called Mental Health Crisis Intervention Pay equal to 1.0% of base salary, which pay shall be provided on all hours the employee is in a paid status. This pay shall be in recognition of the fact that the City's sworn personnel are routinely and consistently exposed to uniquely hazardous conditions arising from law enforcement interactions with the mentally ill and are required to complete training intended to enhance sworn personnel's ability to perform the hazardous and specialized mental health related law enforcement duties which Berkeley's sworn personnel are consistently and routinely asked to perform, including arrests, potential arrests and addressing other mental health related disputes. Effective the first full pay period of January 2016, the amount of pay shall increase to 2.0% of base salary.

Hazard Premium Pay a. Hazard/Premium Pay in the amount of $1,700 is to be paid in a lump sum to each bargaining unit member for prior service provided during the COVID pandemic. If a bargaining unit member was hired in the calendar year of 2021, such payment would be pro-rated upon the date of hire based upon 365 days. To be eligible for this hazard/premium payment, the bargaining unit member must be on the County payroll as of the date of payment. This payment would be made by the County to bargaining unit members on or before February 4, 2022. b. If in any calendar year of this Agreement (2022, 2023 or 2024) Shawnee County is under an Emergency Declaration related to COVID for 240 days or more of said calendar year, a bargaining unit member shall be paid $600, in a lump sum, by the County within two (2) paid periods of the County reaching 240 days of such an Emergency Declaration status. The calculation of Emergency Declaration days for each calendar year of the Agreement shall be cumulative, not consecutive, and the 240-day threshold may be attained by Shawnee County declaring one or more said Emergency Declarations. i. By way of illustration, if in 2022 Shawnee County was under an Emergency Declaration(s) related to COVID, with the 240th day of said Emergency Declaration(s) being November 1, 2022, then the County would be required to pay each bargaining unit member $600, in a lump sum, within two (2) pay periods of November 1, 2022. ii. By way of further illustration, if in 2023 Shawnee County was under an Emergency Declaration(s) related to COVID for only 90 calendar days, then Shawnee County would not be obligated to provide a hazard/premium payment to the bargaining unit members for the calendar year 2023. iii.   • Insurance Reimbursement If you have a health insurance policy, it will usually provide some coverage for mental health treatment. I will facilitate your receipt of the benefits to which you are entitled including filling out forms and speaking with insurance representatives. You will be held responsible for full payment of our agreed upon fee should your insurance company deny benefits or should your coverage lapse. Therefore, it is very important that you find out exactly what mental health benefits your insurance policy covers. Read your plan carefully and call your service representative if you have questions. Many insurance plans require advance authorization before they will provide reimbursement for mental health services. These plans often are oriented toward a short-term model and provide only a certain amount of sessions per year. Many insurance companies may only authorize a few sessions at a time and I will need to periodically call them to authorize additional sessions. When I call to authorize treatment or continue our sessions, I will provide them with the minimum amount of information needed, usually including a diagnosis, goals for treatment, and a brief summary of your current functioning. It is possible, but very rare, that they would require a copy of my clinical record. This information will become part of insurance company files and is likely to be computerized. All insurance companies claim to keep such information confidential, but once it is in their hands, I have no control over what they do with it. In some cases, they may share the information with a national medical information data bank. By signing this Agreement, you agree that I can provide requested information to your insurance carrier. If you request it, I will provide you with a copy of any report that I am asked to submit. I make it my policy to inform you along the way of where we stand with your insurance company and what kind of information they have requested. Should insurance coverage end for some reason, we can discuss an out-of-pocket session fee. You can always choose to select this option and have the right to pay for my services yourself to avoid the complexities of the insurance industry.

  • REINSURANCE PREMIUM The YRT Reinsurance Premium for each coverage shall equal (i) x (ii) x (iii) / 1,000, where:

  • Weekend Premium An employee shall be paid a weekend premium of one dollar and forty-five cents ($1.45) per hour for each hour worked between 2400 hours Friday to 2400 hours Sunday or such other 48 hour period that the Hospital may establish. If an employee is in receipt of premium payment pursuant to a local scheduling regulation with respect to consecutive weekends worked, he will not receive weekend premium under this provision. Effective June 28, 2005, the weekend premium shall be increased to $1.55 per hour.
