Dispensing fees are capped at $6.00 per prescription. Chiropractic – a maximum charge for each visit not to exceed the schedule of fees approved by the Association of which the practitioner is a member, and where there is no approved schedule of fees, the charge must be reasonable. The maximum is $300.00 per person per calendar year. Hearing aid provision – $300.00 every thirty-six (36) months. Orthotics – $500.00 Orthotics / Orthopaedic Shoes/Boots -- annual maximum, reimbursement to be determined by the carrier for those deemed to be medically necessary. Physiotherapy – Annual maximum -- $1000.00 Vision care – $375.00 every 24 months – includes prescriptive lenses, frames and eye examination by optometrist. *For the remainder of the term of this renewed Collective Agreement, the benefits in Chiropractic, Vision Care and Rider 3 (orthodontics) only will be adjusted to match any improvements to benefits subsequently negotiated by the City of Xxxxxxxx and CUPE 5167.

  • HEALTH PROGRAM 3701 Health examinations required by the Employer shall be provided by the Employer and shall be at the expense of the Employer. 3702 Time off without loss of regular pay shall be allowed at a time determined by the Employer for such medical examinations and laboratory tests, provided that these are performed on the Employer’s premises, or at a facility designated by the Employer. 3703 With the approval of the Employer, a nurse may choose to be examined by a physician of her/his own choice, at her/his own expense, as long as the Employer receives a statement as to the fitness of the nurse from the physician. 3704 Time off for medical and dental examinations and/or treatments may be granted and such time off, including necessary travel time, shall be chargeable against accumulated income protection benefits.

  • Extended Health Care Benefits 12.02(a) The City will provide for all employees by contract through an insurer selected by the City an Extended Health Care Plan which will provide extended health care benefits.   • Extended Child Care Leave ‌ Upon completion of maternity, adoption and/or parental leave, including any extension to such leaves, a regular employee will be entitled, upon written application, to a leave of absence without pay to care for the child.
